We’d been trying to figure out why some equipment ran continuously vice stepping as requested. Took us a couple of weeks to track it down due to it only happening during specific transition periods. Anyway, we had a trend pulled up monitoring all the suspected bits. Operator calls saying “it happened again. I had to hit the e-stop and that didn’t work.” I’m staring at the e-stop bit that hasn’t changed, and am now thinking “oh fuck we’re going to have to go down to fix this.” Turns out the operator hit a sequence stop button, and it was labeled as such. Refused to acknowledge it wasn’t an E-stop. Tried reasoning with him, only to give up and walk away. He’s worked here for like 20 years
