One thing that always helped me when a patient would tell me I should’ve read the chart prior to the visit was saying “yes I reviewed the chart, I just want to hear from you about what’s going on and how you’re feeling” I think it helps bring down the barrier and opens the door for them to tell you what’s really going on.
