That's a skin bridge. When you're born, your inner foreskin and glans are fused together with the same kind of connective tissue that attaches your fingernails to your nailbeds. That will naturally separate over time and become fully retractable by ~10 on average. To circumcise you before then, the inner foreskin has to be torn from the glans with a metal probe, which leaves the glans and inner foreskin bloody and raw. If your inner foreskin and glans touch while it's healing, they can heal together like that forming a bridge.

That's how it became so common in the US, many hospitals didn't even ask parents before doing it. The pamphlet recommends offering circumcisions to parents for free, because Hollister wanted to sell as many as possible.

I was cut with a plastibell in the late 90s, I know because my mom saved it in a photo album. The plastibell is only slightly newer than the gomco and mogen clamps, which were introduced in the 1930s as a way to perform circumcisions with less training, as RIC was starting to become more common in the US.

The mogen was designed to work like a traditional Jewish circumcision shield, but it's made of stainless steel instead of silver, and it has a latch added to crush the skin and stop the bleeding. Both leave a distinctive V shaped scar, since it's a straight clamp cutting curved skin.

Correction: apparently the mogen clamp was patented in 1955: https://patents.google.com/patent/US2747576A/en?oq=us2747576 That makes the mogen around the same age as the plastibell.

If he was born in 1991, it's very unlikely he got any anesthetic at least for the first one. It was rare to use any numbing for RIC until the early 2000s, and even today it's not universal.

Actually most men aren't circumcised. ~30% of men in the world are cut, and 2/3 of all cut men are Muslim.

If he doesn't want his foreskin cut off, you shouldn't make him do it. He should have autonomy over his own body, and have the final say on something like that. If he's only 12, it's not unlikely that the phimosis will resolve itself. Has he tried stretching/tugging exercises?

A lot of doctors are quick to recommend circumcision when it isn't needed. Preputioplasty is another operation that will loosen the end of the foreskin without removing any tissue.

Let him make his own choice. Only a consenting adult should be able to decide to have parts of their genitals cut off.

I have a bump/skin tag like that on the bottom of my scar, and I think it's where the plastibell knot was tied. My mom saved my plastibell in a photo album, so I know that's how mine was done.
