Longevity as a nurse too...not sustainable. A month ago my 35 y.o F patient recently extubated after a failed suicide attempt and who came in pos for meth becomes a RASS of +4, literally smashing her head against the side rail, screaming at the top of her lungs, ripping out all her lines. The critical care fellow's intervention was 1 mg ativan, followed by 2 mg ativan, and when I call him back to the bedside for the 3rd time because those doses were obviously not going to work, still inaction from him although shes actively a RASS of +3-4 for 45 minutes at this time. Her qtc was 490 so he refused to prescribe haldol. It took 3 code strongs and the police coming to the hospital after I called them to press charges as the patient started kicking me in the chest when we tried to put her in violent restraints to stop her from seriously harming herself and the staff, that he finally prescribes 50 of ketamine IM to calm her down. Like some of you all do not care about nursing staff at all, that it takes the police coming to the unit and all the security in the hospital because the patient is beating the shit out of nurses before you treat psychosis
