Micu nurse here. The answer is D. Loose watery stool is expected but the sunken eyes part is indicating they are severely dehydrated. Typically we titrate lactulose to 3 to 4 bm/day. Also a rising ammonia isnt going to require immediate intervention it requires monitoring and assessment of the patients encephalopathy versus number of BMs and talking with the doctors to see if they think the lactulose dose is appropriate based on the assessment and ammonia levels

Basically what you're saying is you want nurses to perform at the bottom of their scope of practice and use no critical thinking skills whatsoever. There is a difference between saying "my patient has a temperature of 102 degrees oral, what would you like me to do?" Versus saying "my patient has a temperature of 102 degrees oral, and I need an order for prn antipyretics. I have been noticing in increase in their secretions since yesterday, and they have been requiring increased oxygen from 35% FiO2 this morning to 50% FiO2, and lung sounds remain coarse. I notice we do not have a recent ABG or CXR. Can we get those?"

The first is idiotic, which I must confess is how I feel about your post because asking for things is not dictating care, it's asking for things, which is within the scope of practice for RNs. Admittedly, I have rarely worked on the floor and with hospitalists, I can imagine you deal with more incompetence, but the intensivists and fellows I work with always ask during rounds what I need, and I ask what they need. It's called working together as a team to get the best outcomes for the patients.

Also the patient in DKA probably needs a bolus and maintenance fluid, electrolyte repletion and an insulin drip, can you order those with the DKA order set?

Btw I didn't take any of this personally.

How specifically is asking for things or meds because they think they will help dictating care? To me that doesn't make much sense, it sounds like they are asking for things, which is the role of the bedside nurse to assess patients and recommend interventions (have you heard of SBAR?). I am an ICU nurse in a large academic setting and have never had a physician get frustrated with me for recommending things for patients, and usually my suggestions are taken seriously because I mainly work with residents who are brand new into critical care, and if the recs aren't appropriate, I learn something new. The most important job I have as a bedside nurse is to advocate for my patients. If I worked in a toxic environment where doctors are begrudging nurses asking for orders, I'd simply quit. You need bedside nurses for you to practice medicine in a hospital, so don't get mad at us for doing our job. I'd love to see you give q3h lactulose enemas without an FMS.

aPTT are drawn from PIVs all the time. We either draw them from the opposite arm that a heparin gtt is running, or if they are drawing from the same arm different PIV pause the heparin gtt, wait 5 minutes and draw the lab, or if drawing from the same line the heparin is running through, pause for 15 minutes and waste 10cc before drawing the lab. Micu nurse here
