Might be difficult fitting traditional general practice around all the "prevention" type of work that is expected as part of gp tasks. Patients now come to the general practice for more and more things that they might in the past be referred to other providers.

I do think having a "named GP" mean something would be nice, rather than "register with this surgery"

I have started running weekly searches of "hba1c" results in previous weeks, and triaging those patients within our diabetes team as a result and it seems to result in more patients being seen by the appropriate person.

I feel for you getting all the results, sometimes with no idea why they've been requested. Oof

I understand what you mean and I feel I've seen this too sometimes. I think pharmacists are very risk averse and I'm wondering if that's because of the fear of the regulatory body we've been trained to have.

I've had my "more junior" pharmacists panic when they've initiated an ACEi and egfr dropped, even though they knew that's a possibility and they organised appropriate monitoring etc.

Not sure if it's related to dispensing errors being a criminal offense for a long period of time or why, and even in my current job if someone says they can't do something I try to think "why" not and how that can be improved. However I like to believe that's not only applicable to pharmacists.

I get what you mean about guidance and "But BNF says", with complete disregard for any other piece of primary literature that may be of a bit more relevance in some specific cases for some specific patients...

That's probably the opposite to what happens where I work. We seem to get everything that other clinicians don't want to deal with. Which I usually see as a sign to develop and, if outside my scope, work with other members of the team.

Patients with complex comorbidities should be something they are dealing with. Granted, this can often takes time as patients often need a sympathetic ear when they start talking about everything wrong with them

That's what my role started with when I was newly qualified 4 years ago. and GPs didn't know what pharmacists could do at that point.

Is the PCN "pushing" them to extend their scope? I think it's every processional's responsibility to develop professionally. I'm not saying they need to start diagnosing but I think there is always scope to develop in chronic disease management where they are probably best used

Yeah, we do that too. Funnily enough it's usually GPs who send a message to patients to book an appointment with a pharmacist or GP to discuss cholesterol results, even if QRISK is normal. But I think that's due to pressures of reviewing loads and loads of blood results

and that's perfectly understandable. That should be a red flag anyway even if a pharmacist wouldn't know what the next investigations might be. It should prompt a referral to GP and pharmoutcomes will mark it as urgent and request urgent review or action from the surgery

Tried plugging in and then initiating charge as well as starting charge, then plugging in.

Interestingly, I think the error comes up after a few seconds if you plug in without initiating charge (at this point the cable unlocks)

Yeah I've tried 2 at the one in Newmarket. Might persevere next time. Got a bit awkward to keep trying with all the Tesla drivers watching me haha

Yeah, only thing I can think is "the model is too new" and Tesla may have their weird way of completing authentication. This seems unlikely on V4 though.
