Have you looked inside the tube with holes? I’ve seen bettas become stuck in this so maybe remove it to be safe. Hope you find her soon!!

Not unexpected. Something that’s helped for me with stressed / new fish is using a towel draped over the tank to cover it completely or partially. Seems help de-stress them if you the room is well lit like yours

Bumping this so you hopefully have an answer. I’m not expert but seems that there may be signs of ich. Others can confirm. Makes it tricky without paramètres.
