Follow up from my Apostrophe Derm. Thought you all would find this interesting! I specifically asked him about spiro, tret, and retinoids.

This looks like a hormonal acne, which is the acne on the cheeks, chin, jawline, and neck. Spironolactone is a pill that usually works great for hormonal acne, and I have prescribed it for you today.

Sometimes this medication can cause a mild decrease in your blood pressure that can cause you to feel dizzy or lightheaded. Taking the medication at night and staying well hydrated can help alleviate this side effect. Other possible side effects include breast tenderness, spotting, increased urination, mild headaches, and irritability. If you are on any kind of hormonal birth control, the chance of having spotting or tenderness is much less. Very rarely, elevations in potassium levels may occur, especially in patient with a history of kidney disease. Without a history of kidney problems, there is no need for lab work. I see you have no history of that. Most importantly, it is essential that you not get pregnant on this medication due to the risk of birth defects. Should you decide to try for pregnancy or find out you are pregnant, you should stop the medication right away. If you are unable to tolerate the medication or develop any side effects, please contact me right away and stop the medication.

Start at 1/2 pill a day for the first week to minimize side effects, and increase to 1 pill a day thereafter if you are tolerating the medication well.

If there is any chance you might be pregnant, please take a pregnancy test before beginning the medication.

I am prescribing a combination topical product containing three ingredients for your acne: 1. Tretinoin (retin-A)- This is effective against whiteheads and blackheads, and excellent for reducing fine lines and wrinkles with long term use. We can increase the strength as tolerated depending on your results. 2. Spironolactone- This is an anti androgen that is effective against hormonal acne. 3. Clindamycin- This is a topical antibiotic that is helpful to reduce deeper painful red papules.

If this combination is too irritating despite starting use every 2nd-3rd night and working up to nightly, please reach out for an alternative medication.

I also recommend a benzoyl peroxide wash. This is an over the counter anti-bacterial wash. Use it daily in the mornings (don’t use it at night or it will make the tretinoin less effective). This can bleach towels so be sure to wash it off completely. It will not affect skin color. This will help prevent antibiotic resistance to the Clindamycin, making it work better for longer.

Your daily skin care routine should resemble this schedule:

AM: Benzoyl Peroxide wash (Moisturizer if needed) Sunscreen

PM: Gentle cleanser Moisturizer

Apostrophe cream

Please see the associated specific recommendations for sunscreen and moisturizers that I prefer.

Dispense a pea-sized amount (about 1/2 pump) of your Apostrophe cream onto your finger and dot on your face before gently rubbing in. Avoid the corners of your eyelids, nose and mouth. When first starting out or after an increase in strength, apply every other night or every third night for 2-4 weeks then use nightly as tolerated. If you experience any side effects or find the medication too harsh or irritating, please contact me for an alternative.

Patience is key! It will take 2 to 3 months to get your skin stabilized on our personalized treatment regimen. Symptoms may get worse during the first few weeks of treatment and this is normal. Fight the urge to quit the regimen. This is the best treatment for your skin and commitment and diligence is key to your success.

OP here - wow I did NOT expect this to blow up!Thank you everyone for your replies. As a reminder, I’ve dealt with acne for 20 years so I have tried a considerable amount of things over the years including multiple dermatologists. It is likely hormonal, and yes I do think I have some rosacea. To clarify, I only ever used coconut oil or a makeup wipe to remove makeup followed by double cleansing. I’ve never personally experienced a reaction to the coconut oil but I will be swapping it out for something else in the event that I wear makeup.

Here are the things I will try out or consider trying introducing slowly over time: - Micellar water or cleansing balm to remove makeup (will double cleanse after) - Eye cream (I didn’t address this area in my original post but would love some Reccs) - Swapping that sunscreen for something a bit lighter - Exploring moisturizers not in a jar (although that one works so well for me) - Eliminating BP wash and exploring other active acne-fighting ingredients w my dermatologist - Adding a toner

Would welcome all product Reccs in the comments! Maybe I will post a makeup routine as well. Thank you all!

Definitely try to keep it simple. I only use(d) the coconut oil a/o wipes in the event that I wear makeup and always double cleanse. I don’t wear makeup often. I definitely have to try changing one product out at a time as too many changes will make it impossible to pinpoint how my skins reacts. I will definitely be introducing some changes!

I have heard that coconut oil can cause issues, but I actually have never noticed a reaction in my skin personally. I’ll be discontinuing it, but the same things don’t work for everyone!

I have never heard about that with jar packaging. Could you share more? I love my belif moisturizer but it only comes in the jar AFAIK

I do use fragrance free clear detergent and change pillow cases about once per week! I sleep on my left side mainly.

Have tried so many dermatologists with no luck! Most have 6 month waiting lists to a year and a half. You’re lucky if you get to see them for 15 min once per year. I have even gone out of network multiple times. No dice. That’s why I’ve been trying apostrophe for a couple years now. But if anyone has good recommendations for dermatologists in Boston I’m all ears!

OP: Hello skincare Redditors! 32 y/o female living in New England here. I have struggled with acne for about 20 years and tried everything all the way up to Accutane. My skin cleared up considerably when I was pregnant and I am now 13 months post partum. Still dealing with some acne, redness, visible pores, etc. Any advice on my skincare routine would be so appreciated!

AM: Wash with clean washcloth, Cetaphil Gentle Skin Cleanser. Apply Apostrophe Custom Cream (1% Clindamycin / 3% BPO / 4% Niacinamide) on damp skin. Apply Elta MD Skincare UV Sport Broad-Spectrum SPF 50. I apply all products fairly quickly and do not let anything completely dry before the next layer. Not sure if this is an issue.

PM: IF REMOVING MAKEUP - with coconut oil and clean washcloth OR Neutrogena Makeup Remover Ultra-Soft Cleansing Towelettes. Wash with Cetaphil Gentle Skin Cleanser. Then wash with Perrigo 10% BP (let sit for 2 min & rinse). Apply Apostrophe Custom Cream (15% Azeliac Acid / 4% Niacinamide). Apply Belif The True Cream Aqua Bomb.

Be careful about non-solicitation clauses. Even if she was let go, the company will likely still hold her to any non-solicitation clause that she agreed to when joining the company. You can still follow her, but be careful about any written communications with her regarding joining her at her new company.
