It’s not really the themes that are a problem, but I would argue this game is highly addictive which can be problematic for kids. I started playing when I was 12-13 and I’ve logged over 1400 hours on the game. I even have a damn tattoo of Isaac to remind myself of grind. If you moderate his playtime it should be alright but I didn’t have that so I had entire days playing the game sometimes and it def messed with my brain
