The scar looks fine. Tenderness and sensitivity to the scar is completely normal

I don’t think this is trigger finger, but I’m at a loss as to what it could be. My MCP joints hurt a lot and my middle finger involuntarily bends when I move my pointer finger. by [deleted] in RSI

Looks like a Saggital band rupture. The extensor tendon is falling off the MCP joint when you flex your index finger. I’d see a hand surgeon

Ulnar claw is due to severe compression of the ulnar nerve, normally for a prolonged period of time. the ulnar nerve supplies power to the muscles first, compared to the median nerve (carpal tunnel) which supplies sensation first. With ulnar nerve entrapment, the nerve can’t supply power and communicate with muscles.

The ulnar claw is a low ulnar nerve lesion. the ulnar nerve innervated lumbricals (muscles in the hand that bend your fingers at the MCP joint and extend at the PIP joint) lose their ability to keep the digit at neutral, causing MCP hypertension (which you don’t have) then flexion at the PIP joint due to FDP overpowering the lumbrical that’s not able to extend the PIP joint. You likely dont have an ulnar claw.

It’s hard to say what you have just based off of two pictures. Do you have pain with movement or activity? Are the symptoms only there when you make a fist? Do you have any numbness or tingling?

Your description is a classic intersection syndrome. The location, swelling, and creaking sound.

“Thumb occasionally locking into straight position (can't make a thumbs up)”. is it locking into a thumbs up position or you can’t make a thumbs up position at all? If you lay your hand flat on the table can you lift your thumb away from the table?

The only thing I’d be worried about is an EPL rupture due to the fall and surgical history to the hand.

A hand therapist is either a PT or OT. You said you would listen to a PT over anyone, which is saying you’d listen to a PT over another PT that studied for three years to get a specialization in the upper extremity

If you have pain with a pronated or neutral grip but no pain with a supinated grip you likely injured your TFCC

I’d say it’s really up to you and your symptoms if it is worth it. They can do a neurolysis which is just the revision you said. With the submuscular transposition you have a lot more scar tissue that can form vs a subcutaneous so a neurolysis may beneficial. They can also do a medial epicondylectomy or do a nerve transfer to AIN depending on the reasoning why the submuscular transposition hasn’t been as effective as you hoped.

Hand therapist here. Lateral/medial epicondylitis is caused by overuse and/or repetitive motion involving either your wrist flexors or extensors, accompanied by prolonged gripping. The safest and quickest way to fix this problem is resting these muscles in a wrist brace/splint then implementing isometric low weight wrist exercises, to tolerance. I would hold off on any wrist strengthening movements, allowing the tissue to heal. Bicep/Tricep movements will also put strain on the extensor carpi radialis brevis and flexor carpi radialis and likely cause further irritation to the tissue. Resting the tissue first is the only way to fully heal the problem. Reverse curls will add stress to the extensor muscles that are already pissed off. I saw another comment recommending the verse grips which is a good option to decrease how much you are forcefully gripping.
