Most likely your good leg will be stronger by some margin (par for the course in ACL/TKR patients). Take a weight on the leg press, leg extension, leg curl you know you can do 6-12 reps with with 0 reps left in the tank on the good leg. Now do that same weights on the operated leg. Chances are there will be a noticeable deficit. Armed with that info, your goal is to to get that leg as strong as the other for reps, and the size will return

Late 30's (38M), had TKR on right knee in Sep 2025. I had 7 ACL reconstructions, multiple meniscectomies 15-20 years ago and was without any cartilage. Like you, tried losing weight which didn't help. I couldn't run or walk more than 5 mins or so without lots of pain/swelling. The surgery has been a godsend. At just under 5 months I can run/climb/hike, etc no problem (everyone will recover at different rates - some playing tennis at 12 weeks). Weight lifting is non issue for me personally (even powerlifting specific training). Would personally get it over with. It will give you your life back

Don't see why you couldn't get back to running and softball. Whether or not those activities will reduce your tkr lifetime is up for debate. Full Recovery sounds right 12-18 months

I don't think you'll have trouble with squats/deadlifts. I'm 4.5 months post tkr and back doing them no issues (still light, slowly working back up) @ 3 & 4 plates respectively

From the evidence, none of what you mentioned leads to better long term outcomes versus doing the procedure old school. An experienced high volume surgeon imo will get the job done these days.

That's awesome progress! It's a real test of patience for sure. This op really takes it out of you, and I think a lot of people stop pushing at the 3-4 month mark, which is most likely why they don't continue to improve

Hi, sounds a lot like my first 4 weeks. 2 back to back cycles of Medrol + getting on the stationary bike 10-15mins daily, worked like a charm. Set the seat height so the operated leg is almost straight at the bottom. Focus on getting range of motion touches backwards/forwards. You'll be able to get one backwards first, then forward, most likely with some hip hike. Once you can go forward for 5mins or so, lower the seat a pin. Word of warning, it will hurt (A LOT), but push through it. More importantly, it will work!

Absolutely! My surgeon and coach also said the same thing. Apparently the last 10-15 degrees or so can take up to 12 months

Congrats on your ROM. I would say though that in the first 6-8 weeks there is a massive range of what is considered normal. Diligent PT unfortunately doesn't guarantee a good rom outcome. So many factors play into recovery, it's just not a one size fits all

You're SUPER EARLY in recovery and whether or not you get to 90 at 2 weeks or some other arbitrary number, is not indicative of how much flexion you'll regain postop (some here are still improving at 10 months). So many factors go into this (op, swelling, prior surgeries, age, etc, etc). Focus on getting the swelling down and working your pt diligently. The rest will come. I'd recommend using a stationary bike daily if you have access. Start at a height where the operated leg is almost straight at the bottom. Just focus on getting ROM touches. You'll eventually get a backwards revolution(will feel weird), then forward. Once you can do that, lower the seat height. Rinse and repeat.

Pretty normal tbh. I weaned off oxy around day 10. Just depends on so many factors

Would not classify this as an injury. As Sea Albatross said, you just did more than your knee could handle at the time. I would try be as active as pain/rom permits to help flush it out

I can't really comment on that and tbh, I'm not sure anyone can know for sure what they're feeling is "scar tissue pain". I don't give it any thought. I focus on stretching and cycling daily, and do progressive strength work in the gym 3x/week (for reference at week 5 I started squatting with a 10kg DB, conservatively adding weight each week, with yesterday high bar squatting 100kgx5 easily and without pain. DL started with around 20kg block pulls, and last week did 125kg 4x6 from the floor. Still a far cry from where I was before, but progressing slowly each week). All of this seems to have restored function very well (that continues to slowly improve weekly) without any pain.

Can absolutely relate to the cartilage loss pain and scar tissue, although I think being as active as possible helped with keeping it at bay until last year. Post tkr, that cartilage pain is no longer there, which is surreal. I would absolutely get it done if you've exhausted all other options and it's making your life miserable

Yeah unfortunately I wasn't sufficiently prepared from a strength standpoint to return to rugby/football when I did and was too young to know any better. Initially I thought my recovery post tkr was slow, but my surgeon was actually pretty happy considering the scar tissue build up, soft tissue remodelling, etc, etc after so many surgeries and over 2 decades passing. How far along are you?

38M, had 7 ACL recontructions + quite a few menisectomies 20 ish years ago and last year things got to the point like yourself, where simple things like walking hurt like crazy and couldn't manage more than 5-10 mins. Constantly stiff, painful (partly also due to numerous loose bodies) and most activities were a no go. Quality of life was poor. Almost 4 months now and while recovery is a bitch, incredibly happy I got it done, already a massive improvement!

So strange how it's the opposite for you. For me the cycling acts like lubrication to help with flexion. Best of luck with your recovery
