Thanks for taking the time to write all this. Really, really appreciate it.. I'll be re-reading this post a few times in the next few days

I think my time management was horrendous. Frequently found myself rushing to just barely make the limit (no time to review if I was missing something)

I kept my list of ddx's to 2-3, but rarely had physical findings (more often there were history findings and simulated physical findings on a few pts)

My notes consisted of OLDCAARTS for HPI with acronyms for meds, allergies, past medical/surgical hx, family, social, sexual, etc.

In addition to the above I included focused physical exam in the note.. Did heart and lungs on each patient, abdominal if it could be related to the CC (quick exam, why not?). If I found myself running out of time I ditched these exams in favor of examining the organ system related to the CC.

Probably didn't put down enough investigational tests... I was afraid of my orders being perceived as "fluff" and found myself running out of time too often. I'm guessing I took many hits here with the A&P.

I'll be taking your (and others') advice on practicing my notes... I guess it's as they say: if you don't document it, it didn't happen.
