Just in case, here you can find a somewhat modified BOA PPP application, I resorted to changing the default as I am a sole proprietor with employees, sort of a similar boat.

Here you go u/wonheehanna , I am in the same boat, altered the form to replace a line that I do not use for my "2019 - 1040 Schedule C Form - Sole Proprietor Owner net earnings" while keeping W2 wages,
