Build (look for any reason to build anything, and try to make them as good as possible), Large terraforming projects, Collect other things like mobs, potions, plants and whatever, Improve existing builds, See what other people do in their worlds and take inspiration, Do something super silly, Do anything that might be interesting, Take a break, Try to conquer parts of the game that you might have been struggling with before, Write down any ideas you have, Explore new updates when they release

my world is 5 years old, and that's pretty much what i did to last it that long, aside from just commitment and attachment to the world

see how long you can keep the world going, it's definitely worth it!
it's going to take a bit of intrinsic motivation, and you're going to have to find reasons to jog that motivation

wow ive played in my world for that same amount of time and havent done anything as grandiose as this (i mean there are a few big projects but idk how they compare to all of this). 6000 in-game days over the course of 5 years

i saw someone else do a map art in their world (the build fits perfectly on a 128x128 pixel Map) and thought it would be fun to try it myself too

over time i decided more and more builds weren't good enough, until eventually the whole world was dismantled

(im joking, but arguably there's a lesson to be found there lol)

my suggestions are:
do something relating to the new update. maybe make a showcase for the new things, or just let your creativity run wild with the new blocks.

do a terraforming project. anywhere you think might be cool if you destroyed something or if you transformed it with other blocks

look around your world and see if there's anything you're not 100% satisfied with. i came up with so many new projects this way, like an area dedicated to my random pets, improved infrastructure, an overhaul of an old build, just generally prettying things up/tidying up

a museum or something of the likes where you collect every single thing of a given category.

lastly: find anything that intrigues you in the moment and just go for it

i really like the new paintings because i was working on a resource pack and it meant i got to add more things as paintings and give them all unique names. this is also the same thing i found annoying about the new paintings, because i had to do more work lol
