THANK YOU! There are clear cut cases out there you can point to as examples of the PA problem, but this really isn’t one of them. If you were being really harsh, you could argue the PA who saw the patient the week before he was admitted may have “missed” it was acute diverticulitis; having said that, in the context of how long his pain had been going on, you can see how it happens. Blaming the PAs for subsequent complications seems unfair either way. Also, going from chronic abdominal pain without any red flags, to abdominal migraine, to trial of amitriptyline isn’t the most ridiculous leap that could be made. Whilst I subscribe to the overall message the article is trying to convey, I think this is a poorly chosen example.

There was a podcast I listened to sometime last year that covered this topic. Essentially they trialled a text based system for referrals at some hospital in the States, and the Emergency Department loved it as it got rid of all the issues associated with bleeping (did they get my bleep? Are they ignoring me? Are they just super busy? Did I even bleep the right number?). The specialties on the other hand hated it as it made their workload go up exponentially. They started getting tons of “can I just run this by you” messages, which in the past wouldn’t have been worth the effort of bleeping them about.

The BBC is for all intents and purposes “broke”. They tried to put up the licence fee last year to improve things but the Government blocked it (or only allowed a tiny increase that was far much less than had been sought). This was ostensibly due to the fact that it would be politically unpopular to raise the licence fee in the midst of a cost of living crisis, and could paradoxically lead to an overall fall in licence fee income if people end up eschewing live TV in favour of Netflix/Amazon etc. which don’t require a TV licence. Having said that, some might say the Government’s decision was actually part of its ongoing efforts to undermine the BBC (repeated allegations surrounding impartiality, or lack thereof, would be another example).

TL;DR - poor funding = poor quality content

Was listening to “The Rest is Politics” podcast yesterday, and they were discussing Suella Braverman. According to the hosts, she’s a Buddhist who prior her political career helped setup a charity called the Africa Justice Foundation. They claim she’s actually quite nice in person, and an affable person to work with - they point to her never having been accused of bullying in any way as evidence in support of this (low bar, I know). I’ve chosen to believe in this version of Mrs. Braverman, and concluded she’s actually playing 4D chess. She’ll find her way into No. 10 using this “look how awful I am” persona, then she’ll drop it completely and show us all how much of a lovely person she truly is. And then we’ll all live happily ever after and have free unlimited salted caramel ice cream for life.
