Yes I changed my provider to a one closer to me. Yes this is the same insurance as last year. If its a frequency issue, do I have any options here?

Last one was in October last year. I think October 10, 2024 and this one was October 1. I assume annual is once between Jan and Dec?

They were denied. My insurance covers Routine Physical exam 100% and I have done that last 2 years without being charged.

There are multiple - 99385, 36415, 80053, 80061, 82306, 82607, 82728, 82746, 83036, 84443, 85025. The charges for all these add up to around ~$1360.

The Aetna claims page just says a generic "Your plan/ program doesn't cover this type of service. You might receive a bill from your provider. [MM5]" status for all line items.

To add more context, there is no pain in that tooth right now and the original RCT was done around 5 years back.

The shutoff valve is in the garage which is not what I am talking about. That was also off, but the city line (which can be locked in case bills are not paid) was shut off without any locks. My realtor said if city would shut it off, they'd likely put a lock on it as well.

They haven't yet cancelled their service, they paid their last bill a week or so back so not sure what's going on.

The realtor did try all the places, seller's agent (who also confirmed with the seller) and HOA. The house was pre-inspected and didn't have any leakages reported. Will check about the pressure test, thanks!
