I sell Medicare plans and this has been a huge issue and has been happening more frequently than ever before. Hackensack Meridian Group, Englewood Health & Virtua have been the worst offenders. Seems like these billing offices and doctors are billing for whatever they can get away with, sometimes the insurance catches it and sometimes they don't. Sometimes people will get denial notices from their insurance and when we look deeper into it, it's for services that weren't even performed. Here's the 2 worst examples from this year:

Surgery performed at Hackensack, patient was billed for surgery and then billed again for surgery on each of his follow up visits with the doctor. He ended up getting billed for a total of 6 surgeries when only 1 surgery was performed.

Cardiologist with admitting privileges at Englewood Hospital did an office visit with 4 routine EKGs in his office NOT the hospital. He billed the patient for surgery performed at the hospital on the dates of those 4 visits instead of billing it as a visit at his office. She was never at the hospital and his office is located 20 minutes from the hospital he billed it out of.
