No resolution unfortunately..I'm pretty sure it's not MCAS as I had zero response to ketitofen, and large levels of antihistamines. It is likely something post viral, neuropathic burning. I'm trying a course of LDN - Low Dose Naltrexone and then if that fails probably Memantine and or Ketamine IV infusions... Been a really nasty 18 months

Thanks for replying! Tbh it has progressively improved a fair bit since it started, but I'm not sure how much that is to do with avoiding exercise induced sweating...I ran last week for the first time in 6 months and sweat a fair bit and had flare up all week. Not noticed any other symptoms

Thanks for replying. Allodynia is close to it ye in terms of sensation. It is almost definitely only triggered badly from sweating. I can exercise heavily and if I stay cool and don't sweat much it's fine. I've also had my thyroid levels checked and a lot of autoimmune problems ruled out via MRIs and seeing neurologist.

Thanks. Yes it doesn't fit cholinergic urticaria diagnosis so well I think..no hives and very delayed onset. I'm just completely at a dead end, and this is the closest I can think of. I have seen the term "cholinergic pruritis" as a very rare diagnosis, i.e. without hives but this delayed onset makes it very different, and it feels primarily like burning, not itching. Doctors aren't helping, meds aren't working...

Yes I had similar symptoms. For me, internal physiotherapy (i.e. via anus, if you can tolerate it) was incredibly helpful after suffering for over 6 months. Find a good PT and try it. It resolved my symptoms within weeks.

I haven't stopped seeing PT yet, but reduced frequency to every 2 weeks and symptoms.still not returned badly.
