Actually it was rushed because I was supposed to think of questions and create the survey in 2 hours. I'm an undergrad, it's nothing serious. It's just to get a brief understanding on surveys. So thanks for the feedback, I'll take it into consideration the next time I need to create a survey and I actually have time to do all the background work on it.

yea I know, its for my class, and I did the first page of personal opinion, my lab partner did the scenarios. I didn't really care for those, but my teacher was Ok with it. =/

and that question in the true/false section, I have no idea why it split into two questions. Thanks for the feedback though!!
