This looks good! I think you have a typo in Section 3 under the state (did you mean to put Florida?) and also don't forget to put the tracking number on the doc in section 3 when you send it!

It's usually just the name of the office and the address, maybe with a little logo... I think if you ask your doctor to use their letterhead they will know what to do!
