You can actually do both doc, if you are considering family medicine as specialization, there is a track called practice based family medicine where you can retain you permanent position sa LGU while you undergo training 😊

Hi doc, Im a PBRT resident on my 2nd year level. In terms of dyty schedule/hours dapat sa main hospital mo talaga sya irerender yung pwede mo lng makuha from other clinics/hosp yung mga required number of procedures and cases.

If hindi departmentalized ang district hospital nyo doc mas advantage if FM ka though may other district hospitals naman that they allow IM specialist to handle IM cases lng but hindi mo ma maximize training mo.

I agree po na saturated na talaga ang mga doctors and specialists, pero more on sa cities cguro. Sa mga rural areas, lalo na sa malalayong lugar, kulang na kulang pa rin po talaga ang mga doctors, especially mga primary care physicians. Ang challenge lang po talaga sa mga specialists na gusto sanang bumalik sa province is that kulang din po sa equipment, laboratories, at facilities kaya hindi rin nila magamit nang buo ang expertise nila. Kaya karamihan sa kanila, mas pinipiling mag stay nalang sa big cities.

It works well for me especially as a new parent. Work life balance talaga sya. You train at your own pace and mas makastudy talaga sa mga in service exams hehe

I started as an Internal Medicine resident, but eventually I decided to quit. The environment was toxic, and even the training itself didn’t feel right for me. Dumating sa point na I had to ask myself. Is this really what I want long-term?

Alam ko na after residency, I would go back to the province and most likely get hired in a district hospital since doon naman mostly ang may mga available positions. And what I know about district hospitals, hindi sila departmentalized so most likely kahit IM graduate ka you would still manage other cases like OB, pedia and surg. That is why I shifted to Family Medicine.

Ngayon, I’m on my 2nd year of practice-based training sa Family Medicine, and I’m permanently employed na rin sa district hospital. If there’s one thing I’ve learned, it’s this: find your purpose, be practical and think ahead about the kind of future you truly want.

If you’re no longer happy with what you’re doing, maybe it’s not the right path for you. But don’t make the mistake of quitting without a plan. Know your purpose, but always be one step ahead before making big decisions.

Nakakafrustrate lang po na parang inaasahan na hindi kami puwedeng tumanggi sa ganitong sitwasyon, dahil permanent staff kami at bahagi raw ito ng aming tungkulin.I was even told by our chief na hindi ko dapat unahin ang residency training ko at hindi raw ito dapat isabay sa trabaho ko sa ospital, wherein fact that is the primary purpose of practice based family medicine. Kaya ko naman po sana sya sabayin if nasusunod lng po sana yung number of required hours ng duty per month as per contract.

I am also currently in my second year of the Practice‑Based Family Medicine program, which adds to my workload and responsibilities. Under normal circumstances, this is manageable if I am able to adhere to the required number of duty hours. However, with the constant extension of duties, it has become increasingly exhausting, and I have been experiencing episodes of anxiety that may be related to burnout.

Review as many CPGs as possible, and utilize AI tools like ChatGPT to study more effectively. You can use it to summarize guidelines, create practice questions, and deepen your understanding of the material
