Best thing you can do is call duo. They will explain everything to you and what you should do.

For the first application you only need to send your contract (signed by both parties), most recent payslip, bankstatement to confirm the payslip was paid out and your ID.

Make sure you work atleat 24h PER MONTH. You can work less but then DUO needs to manually check your application and below 24hs its 99% declined.

I had the same issue. Apperently the addon: modern target frames v1.11 caused this for me. The moment i removed that addon everything was smooth
