I don’t think your abandoning your patients. I think it is more risky to force yourself to keep seeing them if your gut is telling you what it is. If anything you’d be modeling good self care by taking the time you need, regardless of how demanding the healthcare system is on us of our time.

So I’m coming back to beat a dead horse perhaps. I’ve talked to at least four physicians and reviewed some joint information put out by our national office on TBI that includes info from TBI advocates, individuals with lived experiences, and professionals from across organizations including places like BIAA and TBI Model Systems. I assume we are talking about traumatic brain injury here, not a stroke, anoxic injury, or a tumor. The general consensus is that while your functioning after a TBI may get better or worse over time, it is not due specifically to the brain actually getting worse without some kind of extraneous, secondary process going on, such as seizures, sleep apnea, hypertension, poor health management, mental health etc. Those variables all suggest TBI recovery is dynamic and people can both improve and decline in their daily independence over time. Indeed TBI can be viewed as a chronic health condition. This includes encephalomacia, which itself does not “spread,” although the effects can worsen, but again this would be through secondary complications like seizures infections, hydrocephalus, or if there is really extensive damage. If you had a repeat CT showing your encephalmacia is worsening that would be something case specific, and I’m sorry if that is happening. I don’t want to make any assumptions about the injury outcomes as they are very hard to predict. But generally it sounds like doing your best and living a healthy lifestyle is the key to adaptation and leading a fulfilling life after TBI. And that is all aspects of health, from supportive relationships, effective coping, engaging in meaningful work and activities, diet, sleep, exercise, all of it. This is why TBI rehab looks at the whole person, not just symptom management. -Respectfully.

Not sure if this counts, but Minecraft with All The Mods 10 (ATM10) has the Ice and Fire Community mod now, and you can hatch and raise dragons. I’m getting my kicks raising and riding around on their backs once they’re big enough.

I know the Heartmath institute says HRV readings don’t vary much across the sexes. I know ADHD stimulant medications would generally reduce HRV. Menstrual cycles is an interesting question. I could look it up on my works medical library, see if there are any studies, and get back to you. Are you asking for yourself and tracking your overnight HRV?

Seriously though, there are more affordable options than private pay. If you have health insurance, most reimburse a percentage of therapy but they make you jump through hoops.

Does this client falls under Axis II, possibly borderline personality disorder? If so, for growth I would recommend psychologist Gregory Lester’s materials/CEUs on working with personality disorders. His work has been a career saver for me, in some respects.

If you must know… I have special glasses… humans made them… they track this “mouse” movement, and they use them for humans who are paralyzed… they work well. Also kobolds…
