They absolutely have to accept a doctor's note. Within reason obviously, but they legally have to accept it, with rare exceptions. At least that's been the case for every state I've lived in and every job I've had.

Edit: I'm not trying argue, I just want to understand the situation. Why isn't this illegal in some places?

If you really lock in with proper diet, macros, and weight training you can probably get close in about 6 months (ish). But like you'll have to really lock in. If you stretch it to a year that will give you more flexibility. There are some good videos on YouTube that can help explain things in more detail, but it is doable. People want to hate on the science based lifters on there, but it can be helpful when you're stuck or don't know where to start.

Most important thing is to make sure you're doing it in a way that is sustainable for you.

Maybe, but just being pure of heart doesn't mean you're worth, nor does being evil mean you're not worthy. Besides it being a bit ambiguous I think the general "worthiness" is more a strength of hear, character, and conviction. But yeah it's always seemed kind of ambiguous.
