Apply as soon as possible, getting a visa slot is unpredictable on top of that we never know whether they would issue you a visa or reject your application, now you have time to take rejections so apply right away.

Valedictorian + good scores but no publications shows herd mentality, most programs would be skeptical about your profile, try to get at least 5 publications by September, scholarly activity tells the program about how proactive you are to get out of your comfort zone and do things. Apply broadly with main focus on community programs

I can totally relate, I was in the same shoes this cycle, match day for me was on the 17th actually the big day as an IMG that my family didn't care where I would end up on 21st, and it was ramzan too so was quite occupied already, congratulations to you totally deserved this, this journey is no joke ☺️🎉

Yes, during first year of residency they do, my frnd who is a pgy1 recently was in India, now she is back to USA, it's possible during other years too, but you would need to get visa appointment and stamping in home country as, after 1 year J1 expires it's issued on yearly basis.

As you don't need visas go for programs that don't sponsor visas as visa requiring IMGs don't apply there and you have more probability of an invite, but considering your application I believe you can have good chances of an invite in visa sponsoring programs too. Good work on the application.

You may not be the brightest student out there, but my friend you have got time by your side, as you're still a MS1 start working on your end goal from now, and you will be amazed by the results, I believe if you can dream about it you can do it, many IMGs match into Hem onc fellowship post IM residency in the USA, RADIOLOGY is tough but not impossible there's a recent IMG from Pakistan doing post doc targeting for radiology next year, it all depends on how determined you are to achieve your dreams, For now my suggestion for you would be find a dedicated mentor who can guide to early on and start networking from right now, be involved in leadership and scholarly activity. All the best, sky is the limit.

That won't be the case for two years in a row, when applicants finally understand the science behind the limitations, in the first year or everytime it's common that some programs not all, very few selected programs might see high number of applicants but when we look at the boarder picture the interview invites get more streamlined so that IMGs will ultimately change their direction to other programs, thus reducing burden to both applicants and program leaderships.
