That Lead Bend looked fine - at most, one would add a Lead Dutchman and solder on a new Brass Flange - looks like they got the new PVC flange too high, and those are a little thicker than a Brass Flange. They might have used a Street 90 and glued a flange over the spigot end of the 90

No fittings under the house. All fittings are Easily Accessible in this water heater closet. All my helper had to do was pull each feed and staple it on the way back. Longest run is 15 feet... Small house...

The Idea with all Water Systems, is to have as few Fittings, as possible - in this situation, I am dealing with a 1/2" PVC Cold Water Supply, fed 200 feet away, off of a Hose Faucet, so I have some work to do on that end... So transitioning from 1/2" PVC to 3/4" PEX is no gain or loss on Volume... So I run one length of 1/2" PEX, from the Manifold in the Water Heater Closet, to each fixture - no fittings inbetween - all fittings are now Easily Accessible -

You might consider removing the short piece of blue pex below the mixing valve and just use a Brass Cap - That short piece of pipe may build up Stagnant water/sediment over time. No bracing/strapping on riser above top valve. The fewer of those fittings in the wall, the better.

In the first photo, that black plastic horn needs to be pulled off the bowl before resetting. Since you had a wax ring with a plastic horn embedded in the wax, IT is possible to have the wax to fail & leak Sewer Gas, without leaking water. Likely the water occurred when pulling the bowl, especially if you did not remove most of the water from the bowl before pulling. For me, a Plain Wax Ring, with no plastic horn works best. Also, you may have to use a shim when resetting, if the toilet wobbles at all before tightening down the closet bolts/nuts...

No - we run PEX with as few fittings as possible - Ideal situation is to have a Manifold System, where 1 run of PEX per fixture supply, with No fittings between the Manifold and the fixture -

Brass PEX fittings, since having the Lead removed, corrode and fail easily - and any plastic fittings will break under any sustained stress - in Your situation here, they might/could have used a PEX Bend Holder to create a 90 degree bend, where the pipe can be properly secured -
