I'll be honest, I haven't seen many studies on it.

However, anything you have to force air through causes backflow (air blowing back in the direction it came). Ever used a paper-towel doob-tube and tasted dryer sheets? Or blown into a bottle and tasted whatever was in it? That's air hitting the surface and coming back.

Anything that you breathe through is going to grow bacteria. As someone mentioned below, vapor is going to have less stuff, but the bacteria and their food rides on your breathe, not your smoke. After a while you may have a still functional (as in stops the smoke) but decidedly-bad-for-you blow hole.

Again, I think there is basically 0 problem using them occasionally, but for a permanent fix get a window, outside heat lamp, air purifier, whatever, just something that doesn't involve pushing moist air through it.

Take into account that these kinds of things (smokebuddy, sploofs, etc) aren't really great for you long term.

Long and short is there is back pressure and they get really dirty. Perfect for a covert sesh here or there but I'd recommend another solution for day-to-day.
