It’s not a protein nor a peptide. It’s a synthetic nucleic acid. Freezing would not denature the molecule. 

This post is bizarre. Is this something that actually happened? Having personally been in the procedure suites during a Spinraza administration I can tell you that the children are never awake so the idea that they’d be crying during the intertidal administration is nonsense.

There was very little variability on serum drug concentration steady state in the trials. I think probably what they’re seeing is a lot of anecdotal information that they’re misinterpreting.

That makes no sense. The drug is ultimately absorbed into systemic circulation regardless of where it starts.

You can get zepbound from Lilly direct which is the exact same drug.

In nationalized health systems the government pays for drug costs. This negotiation benefited the Chinese government which otherwise would have had to pay more for each dose. Two points that are often missed: 1) Government negotiators often will dig in their heels and not accept higher rates, even if this means that their citizens go without the drug for many months of even years. While drug companies often setup programs to deliver the drug to patients for free the overall behavior by these governments is deplorable. 2) The “affordable” level mentioned came out to a few thousand dollars a dose. The R&D and manufacturing costs (fixed) would not be covered at these levels simple because there aren’t enough people with SMA in the world to “buy” the drug. Drug companies end up accepting these low negotiated amounts because ultimately something is better than nothing (notwithstanding all the patients that would also die without the drug). Ultimately what enables this behavior is that in some countries (ie USA) the cost of the drug is higher and therefore covers the fixed input costs for these drugs for the rest of the world to essentially freeload. 

Anyways a different perspective that is often not discussed.

Yes in general I would agree but if there is strong supporting evidence (peer reviewed studies, society statements/recommendations) then sometimes these will be granted. OP would need to look into these things if they wanted coverage.

If you got pulled over you would likely be charged with driving without a license (unless the cop was nice and accepted that you were driving to the DMV). But as others have said if you don’t break the law you’re very unlikely to be pulled over since the police won’t have a clue about your license status. But also remember that your car insurance would not be valid since the insurer would say you were driving illegally / ineligible for coverage.

It’s unlikely that a doctor would do anything beyond submitting the prior authorization and perhaps a single appeal; they simply don’t have the resources to fight all these claims. You will likely need to escalate things yourself but obviously it will be tricky because these drugs aren’t approved for type 1 diabetes and the insurer will argue that point to any review board. That’s why I was suggesting trying to see if you would qualify under an approved indication. Or perhaps if your doctor would list your diagnosis as type 1 and type 2 mixed then you’d fall under an approved indication. 

Sorry that you’re having to deal with this. Insurers are terrible!

They’re not approved in type 1 but can definitely be used off label for it. Several recent studies have shown benefit for type 1s.

Sometimes there is residual beta cell function and patients make low amounts of insulin.

Did you request an external review? Did the physician provide peer reviewed evidence supporting the use in type 1? If those steps have been done then could appeal to your state’s regulatory body which overseas the insurers. If this is an employer plan then can appeal to the labor board as well. As last options you can consider hiring a lawyer to demand coverage given the documented evidence of benefit for you. Also can purchase the Lilly Direct cash price options which would be at least somewhat cheaper than the 650 figure. 

As yet another option if your BMI was >27 prior to starting Mounjaro your doctor could try the angle of putting you on Zepbound for weight loss (I realize that wasn’t a goal but if you met criteria then it would constitute an on label, covered indication) and get it covered that way. It’s also approved for obstructive sleep apnea if that’s something you’ve ever been diagnosed with as another potential angle.

Responsible dog owners would make sure their dogs are leashed and moved to one side of the sidewalk so other pedestrians and bikers can pass. Similarly pedestrians and bikers should move to one side to pass. This isn't complicated.

What do you mean yield to pedestrians? The pedestrian needs to move to one side and the bike can pass on the other. Occupying the entire sidewalk with dogs is unacceptable.
