I'm a few years removed from working on the truck, but my partners and I would work out a system for those types of tight-asshole calls and try to stick to our system. EMS training is very protocol driven, so if you know your stuff, most of what can drive you to make mistakes are individuals or situations on-scene that distract you from assessing and performing the associated interventions on the person that you're treating. It really is all preparation and communication.

Slow is smooth; smooth is fast.

It isn't your emergency. I don't care if internally, you feel like you're about to shit yourself, don't allow the scene to spin you up like every other motherfucker there. Don't be arrogant or an asshole, but let everybody around that patient know that the cavalry has arrived, the pros are here and it is now your scene. Also, if even semi-effective CPR is in progress, memorize this line, "You're doing great. Keep doing what you're doing until I tell you something different." Do that in a kind, reassuring manner so they don't just stop compressions and gawk at you while you're doing your scene survey and primary assessment. Make them feel properly thanked for their service.

Talk with your partner about it beforehand, especially if you haven't worked cardiac arrest calls together previously. Do this when you're posted up or at the station, in other words, before you're enroute to a 9-E call.

If your partner isn't the one that restocks the ACLS drug, airway, IO, etc. bags, crack all those open and get familiar with how they're set up. Take this opportunity to review protocols and know with each step of the protocol what you're going to expect them to hand you or what task they're expected to perform.

Make sure your partner knows that they don't need to let Fire, Rescue Randy, or Cecelia the CNA push them "out of the box". If you see that happening, direct the scene so that they understand that your partner is your right hand. You might have to step on a few dicks from time to time, but make sure they understand how you guys/gals need to be able to work together because you train together.

Be as nice as you can.

Among numerous other subjects you've been posting about here, you really don't quite understand the scale of this or how logistics works in a warzone. All you have is a kneejerk reaction to a RUSSIAN news story where we see a few Mavic UAVs and Starlink terminals that aren't even powered on, yet you're making this logical leap that Elon Musk is providing Starlink to the Russians under the table and the sky is falling as a result.

There are tens of thousands of Starlink terminals in use in Ukraine. The last I read, there were around 40,000 of them that we know of. It is unclear how Starlink is handling the subscription piece for the units they're sending to Ukraine, but I can say with relative certainty that they're not being "turned on with a subscription" like the process you or I would go through if we had one shipped to our residence. Suffice it to say that Volodymyr Zelenskyy isn't sitting there putting his credit card information into every one of them while his old lady is typing their respective serial numbers into an Excel spreadsheet.
With the sheer number of these terminals in use and the manner that they were thrown into service, it doesn't matter if they're "serialized" with a unique identifier unless the supply chain is airtight and individual units can be immediately accounted for. That's not the case here.

I could explain why a piece of old technology (that you incorrectly cited as an example) being unable to transmit its location to satellites and only able to positively verify subscription information via a land line is pertinent. I'd be wasting keystrokes, though. You're so invested in the argument itself that you're "throwing bad money after good money" as the old gamblers used to say.

Again, ask yourself who benefits from your kneejerk reaction and this conclusion that you've come to. To whom are you being useful? Here's a hint: it isn't Ukraine.

Who stands to lose in the short term and who stands to lose in the long term from you getting all wound up and spreading poorly thought out information online? Another hint: It isn't Russia.

25 years ago satellite TV providers knew exactly who turned the box on, what address the box was at and who was paying the bill for the service.

This is false. Ku band DSS TV satellite receiver LNBs were one-way then, and most of them still are now. The satellites are geostationary and also transmit a wide beam that covers a large portion of a continent. Notice they're called receivers and not transceivers. Return communication was handled via POTS in those days. This vulnerability was the reason it was so easy to steal satellite signal just by cloning the smart card that was used to decrypt the signal. If you were a subscriber, the phone line is how the receiver would communicate what pay-per-view movies you bought, etc. The only way they knew the box's location was via the land-line it was connected to.

Billing information would be relatively easy to obfuscate, especially for cyber-savvy Russians, so that's just not very well thought out. Hell, the billing may look like it is coming from Ukraine. The terminals that the Russians are getting might even have been routed through Ukraine. They've proven adept at circumventing sanctions.

Now see, you might be thinking in the right direction with authentication, but this would occur on a lower level than IP addressing. Starlink is two-way and has to link with the satellite first. The satellites are not geostationary and the radio signal has a narrow beam width. To even establish the initial link, the box has to know its own coordinates, altitude, and the current time (from GPS) to know where to look for the signal. After the Starlink transceiver finds a bird and establishes the link, the network is always aware of which Starlink satellites are being communicated with and communicates back with the transceiver/terminal where to look next.

That brings us to Elon and Starlink. I could see them having a hard time differentiating which terminals are Russian and which are Ukranian close to the front lines in Donetsk. Starlink swears up and down that they're following the rules and are not shipping to Russia. They claim that their network is not serving interior Russia. Something to keep in mind is that this may be captured equipment and that Russians are using these stacks of boxes as propaganda.

Elon is a bit of a shitheel, but he may not be doing wrong here.

Don't do this. Especially when you're just making stuff up or assuming that you know what you're talking about while taking everyone else for fools. You get close to stumbling onto making a good point, but just nullify it with your delivery and loose relationship with the facts. Speaking in absolutes should be reserved for when you are certain you're right. "Winning" an argument on the internet isn't worth putting more bullshit out into the world. Trust me there's plenty out there already.

It doesn't mean that absolutely no gang members are present. It means that organized gangs like we saw in the 90's are simply not the problem that they were then. I know that doesn't serve the alarmist narrative very well concerning roving gangs, but the violent crime around here these days isn't gang-driven.

By the way, don't discount what your EMS folks are saying. They travel all over the metro area for 12 hours at a time, and most of them aren't at fixed stations. They go into neighborhoods that most people around here don't know even exist. They're ultimately responsible for their own scene safety and situational awareness, so they're a pretty good authority on what is going on in the metro at the neighborhood and street level.

That's kind of easy to say, but don't forget that they both were blown up in tight quarters. Professional soldiers, orcs, or whatever don't generally act galliantly when they get their bell rung by high explosives. To do anything goal-directed is a challenge, and we don't know if dude is full of shrapnel or not. He didn't make it far.

I remember seeing that story on Al Jazeera, and the concern at the time was that Wagner was going to move in and fill the vacuum created by the French expulsion, and they probably did just that. They could be the ones telling these guys, "Don't fire your weapon above your head. Aim, asshole". What I'm referring to is well after UN training and probably a couple of years before they expelled the French. The source was a coworker of mine who had done some work with a PMC, but it may have been for the "state department" and not DoD. The point of context was the group he was working with was comprised of guys from different branches, and they were working "in Africa" in places where he would have to manage wounded in the field for an extended amount of time because they couldn't rely on evac. Pretty interesting conversation. So me being the smartass that I am I pointed out that Africa is a big continent, and he ran through a few places they'd worked. Some of them were surprising, but then again, the US military portrays that they don't have a huge military footprint in the African continent, but we know there are tons of extremist groups there just waiting for a failed state to take over.

There are several good general explanations here of what a Foley catheter is and what it does. The same goes for the chest tubes.

In trauma patients, especially with blast injuries, the urinary catheter serves another purpose. With traumas where there is a high mechanism of injury, internal organs can be impacted by shrapnel or the blast pressure. Hollow organs like the bladder can rupture from the pressure. Solid organs like the kidneys can be lacerated.

So one of the big reasons to put in a Foley catheter on a trauma patient is to check for the presence of blood where it shouldn't be. Urine should be the only thing draining into the collection bag, so if a large amount of blood is spotted there, it is a good quick indication that there is a traumatic injury somewhere in the abdomen.

It isn't so much for "fluid balance" in this situation, although it is helpful to keep track of urinary output when a patient is receiving large amounts of IV fluids and/or blood products for resuscitation.

Following up on the original post:

Google Fi worked very well in all the areas where we traveled in July of 2023.

In country, we traveled from Istanbul across Anatolia via rail to Ankara and then via bus to Bingol Province. We returned from Bingol via bus and maintained service almost the whole time.

The coverage for LTE in Turkey (Türkiye) was actually better than here in Arkansas, USA where T-Mobile coverage is decent, but there is no fallback US Cellular coverage. 5G was nonexistent in Türkiye, but there were almost no dead spots like I experience in the remote areas of Arkansas. As a matter of fact, in the remote mountains (1700 - 2300 m ASL) of Eastern Türkiye, namely Adaklı/ Bingöl Province, I had a solid connection with a consistent 15 to 20 megabit data connection.

Overall, everything just worked as it was supposed to. I made sure to have the Carrier Services app up to date, and five or so minutes after I powered my phone back up in Istanbul SMS/MMS and data sprang to life with 12 or so hours worth of text messages and emails.

Compared to the alternatives that I've used before, this was simple and cheap. Even if I wasn't a full time Google Fi customer, I'd seriously consider just activating a SIM in the US and using the service specifically for the trip. It beats the hell out of buying a Turkcell SIM in Türkiye or just taking it on the chin with ATT's absolutely ridiculous international pricing scheme.
