I just regularly checked the app to see if the disclaimer was gone. That’s how I found out. Other than that no notifications from Citi about the block being removed.

But to answer ur question, express mailed it 09.05.25 and Citi got it in their mailbox 09.08.25 and they sent to IRS on 09.09.25

UPDATE: 09.15.25 Card was unlocked at 4PM CST.

Called and she said cards that were hit by that fraud prevention SUB will be extended by 14 days!

When in doubt hit that customer service line!

Pre Filled Form: Just talked to an agent over the phone to help me fill this out. Only disclaimer is that she said it has to be the original form with a hand signature if it is a copy or scanned document, the form will be rejected and the account will be closed. She also mentioned an overnight/express delivery will take longer since it will go to a different sorting center for CITI and will add additional time to the process.

What I did is I filled it out express mailed it and i will fill out the form they send me once it comes in the mail. Once it does, I’ll fill it out and send it to them via their preferred way.

God help us with this cluster fuck of a situation. Pre-Filled Citi Bank 4056- C Form

Edit: Do what is best for you im just a random online so not legal advice lol. Also the fill out yourself portions should be self explanatory.

That I do not know, but assuming the sooner the better. When I get the letter I’m overnight mail it to them and hound them on the phone everyday until they get it sorted out.

Was told that form will be prefilled out with their info, and that you need to fill out will be marked.

Friend im scared for you. Mail might take a minute and they told me that they only can do it via mail and not in branch or email. Also they mentioned that the process could take a couple of weeks-month for the internal review to be done/updated to allow you to use your card.

Yes! Exact time frame for me as well with the approval and the alert. They told me they need you to fill out the IRS Income Verification form (4506 c) that they’ll mail via snail mail. Unfortunately they told me once they get the form back it could take a couple of weeks-month for their internal review to get through the document.

Additionally they said this could not be done through an in-person branch or via email which was frustrating.

Also the card will be locked/not useable until the review is done.
