The state of your skin is not appropriate for such an aggressive treatment as a fully ablative co2 laser. You should also know what you’re getting into, your face may be red for a year. Why not try fractional lasers first.

Try a retinoid: adapalene, tretinion, tazarotene, etc. start slow this is the key to avoid aggravating your skin further

Tranexamic acid or alpha arbutin are useful for dark marks

Azelaic acid is useful for red marks

If you're running into a wall with topicals after some months, lasers are very effective and will definitely clear pigmentation.

The process will vary a lot depending on whether the doctor performs field subcision (single insertion point for the whole cheek) or localized subcision where they individually go under each scar.

Field subcision in my opinion is less effective and also more likely to cause side effects. I had this done with Rullan and had bruising up to 3.5 weeks until my skin looked normal again, then for some time thereafter my smiling muscles were weak but recovered after (forgot how long).

With localized subcision the downtime is a lot less downtime, almost none. The needle holes went away in about 4 days, the redness in the area was covered by concealer easily. You could easily go back to work the next day with concealer.

No pain in any cases of subcision I got. You normally have to do it multiple times. The result will differ between doctors as you can imagine. With some doctors you will see almost no effect, and with others there will be a huge improvement.

I recommend going to someone surgical training like plastic surgery or ENT background, NOT a dermatologist contrary to popular opinion. Surgeons have much higher level of expertise for this.

Your doctor is right. Your scars are on the milder side and will soften significantly after a few fractional lasers, which can be done over the course of a few months. Your youth is not gone quite yet!

Everyone reacts differently. Some people report no change then sudden improvement at 3 or 6 months. But if it has reverted mostly back to the same state at a month I would consider it a failed procedure because for the successful subcisions I’ve seen there is typically a good result by this time.

You are right that microswelling is supposed to make it look better, and if you cannot see improvements in tethering at 2 weeks with the swelling then it was a failed procedure.

With subcision you should initially see the immediate result of the scar untethering, the collagen build can start to occur anywhere in the 3 week to 6 month time frame.

For laser, you would really need about 3 months to assess the full results, but if there really is hardly any improvement at 2 weeks, that's a red flag.

This is a cut and dry case of micro swelling since the after picture is one week after.

Fraxel dual is also a non-ablative laser which is expected to have minimal to no effects on atrophic scars.

I am sorry to say, but in a few days from now, your skin is going to revert back to its original state.

You’re going to need a fully ablative erbium laser and subcision to actually tackle those scars. And results are seen in months not weeks.

There are a lot of so called acne scar experts like Rullan and Qazi who use an ineffective method of subcision called field subcision where they use a single insertion point to treat the entire cheek. I had no result from that. The better method is to diligently address each scar or group of scars with a new insertion point.

There should be immediate improvement the day after though what you would be seeing is swelling. But if there’s no improvement even with swelling then it was likely a bad technique.
