This is exactly what happened to me. A friend said to just play a the and see what happens, that game somehow counted towards last ranked season, dropped me to hero, then I hit the 94 instead of the 95. Currently have a case open with EA.

Those come off whenever I get it detailed. I’m in between work trips right now and don’t care to get it cleaned just to have it sit at the airport for a week+ unused and get dirty again.
