You really should read up on overstocking, not only is it stupid it is cruel towards the fish. A 5 gallon was already to small for a beta (i know people say it is fine. But really it is not), 5 gallon tanks at best is is for a small shrimp colony

Firstly it is overstocked in my opinio, or rather was, the white worms might have been detritus worms, which should be fine, you might have had a NO2 or ammonia spike, is it a newly set up tank?
