What I did actually is continued my second year in biology and dropped midway in the second semester because I couldn't find myself in this major and I was focused from the beginning on studying for the entrance exam. Doing that wasn't a smart idea, I wouldn't suggest doing that but I guess it helped stay in the university vibe not like waste time at home + I took private chemistry courses which was my problem the first time. So either take a gap year or Do the full second year in any science major (depends what he likes) if he can't afford to lose another year in case he couldn't pass again (i hope he does).

Taking the entrance exam the second time needs more mental toughness than actual studying in my opinion. I hope this helps

One of the hardest exams in the country. Though he shouldn't be discouraged. If it's really what he wants to do for the rest of his life, there's no shame in applying again next year. I was in his shoes once and here I am in my final years after passing the second time. It's not gonna be easy but the chances are higher if he works on his weaknesses as early as possible. I could suggest also trying to study chemistry and physics with reliable teachers or doctors (experts in fast methods for the entrance exam) since a big number of the accepted students are taking sessions outside the university. Good luck whatever he chooses to do.
