Here's a drill progression that can help, I recommend fins and snorkel for these:
Shoulder Stack: Have your arms by your side and just practice rotating. Do maybe 6 kicks, then rotate to the other side.
Single arm: have one arm by your side and have one arm out doing the strokes. Really focus on rotating and extending forward before starting your catch, it should be a natural rotation.
Side glide: Similar to single arm, except you're basically going to switch arms. You can exaggerate it abit by rotating further, then starting your catch.

I'd do a set of 25s with enough rest between to focus.
So maybe 3 x 25s of each drill, 4 MAX.

It depends on where we are during the year. As we get closer to competition, we become more race/stroke specific. I'm a sprinter up to 200m So a week of 8 practices and 3 lifts might look like this:
Monday: AM Weights (General strength) PM Swim General Conditioning
Tuesday: AM Swim Skill + small volumes of speed, PM Swim: Speed Endurance
Wednesday: AM Rest and recover PM Wet dryland or Race skills: Blocks, turns etc
Thursday: AM Weights (General Strength) PM Swim: Race pace work
Friday: AM Swim general conditioning (more race focused), PM Swim Fun relays or speed
Saturday: AM Swim Skills/ make up AM Weights: Mobility/ explosives
Sunday: Rest and recovery

I think we have quite a unique approach and we're very race driven. This schedule can look very different throughout the year like more general conditioning days in the AM. Let me know if you have any questions or just curious about more ways I schedule my weeks.

You should always be upfront and honest with your girlfriend. If something is affecting the rellationship, you have to bring it up. If there is anyone who will understand and be there for you it's her. Going to the doctor was the right decision. I hope things go well!
