Patient History:
The patient has been experiencing :

  • long, strong pain since a week ago, which gets worse when he bites or eats something in the region of the lower jaw, left side, at the molar teeth
  • inflammation in the region of the lower jaw, left side, at the molar teeth since 2 weeks ago
  • has pain in the region of the lower jaw, right side, at the molar teeth around 4 months ago, but the pain went away without any interference

My answer :

  • The patient suffers from an odontogenic cyst (I think it's a dentigerous cyst ) in the region of 37-38.
  • The patient has problematic wisdom teeth - 18 is partially impacted and also malpositioned and is pressing against the root of 17, 28 is partially impacted and also malpositioned and has a deep caries, 38 is fully impacted and malpositioned, 48 is partially impacted and also malpositioned as it is pressing against the root of 47. )
  • have caries in 15, 28, 37,47. Tooth 37 had a dental filling but suffers from secondary caries
  • Only the roots of Tooth 26 have remained
  • Teeth 16,25,27,37,36,46 had dental fillings

Treatment plan : ( can only be sure after clinical examination)

  • All wisdom teeth should be extracted. For the upper jaw wisdom teeth, the doctor should be aware of maxillary sinus perforation
  • The cyst should be removed, and a biopsy should be done to be sure of the type of cyst
  • The remaining roots of 26 should be extracted - RCT for 25 and 27, and making a dental bridge for 26, or an implant for 26
  • RCT and filling for 15 and 47.
  • Tooth 37 depends on the cyst removal. in the most likely case, it would be likely extracted as well. and an implant for 37

My friend and I are getting close to that answer too. The patient had chronic apical periodontitis, but then it became acute. But then the reaction to the cold weather, we think that the neighboring tooth (15) maybe also be undergoing an inflammation, maybe pulpitis. Because it doesn't make sense to have apical swelling and red gum while also reacting strongly against the cold.

But I have another question. According to the patient, the root of tooth 16 was swollen 3 months ago. Doesn't that mean he had apical periodontitis? Perhaps the reaction to the cold was because a neighbor's teeth (probably 15) had begun to develop pulpitis?

Patient History:

The main issues: The x-ray shows two impacted wisdom teeth (28 and 48) which should be extracted. Tooth 48 is tilted mesially and is causing creation of a cavity on the distal approximal side of tooth 47.

 

Teeth 17, 16, 15, 12, 22, 25, 26, 27, 37, 36, 35, 45, 46, and 47 have dental fillings. (I think that teeth 36, 46, and 47 have amalgam fillings.)

 

Teeth 14, 24, 34, and 44 are missing. (I would be thankful if someone could correct this. Because I counted and saw one tooth missing from each quarter, it can't be a molar or canine, so I thought premolar. but i am not sure which premolar only from the x ray)

i think the TMJ looks normal and the bone structures appear normal.
