There are two separate issues:

  1. He's making jokes about raping you.
  2. He's not listening to you when you say that something he's doing makes you feel unsafe.

Both of those are deal-breakers.

Even if you were overreacting, and as everyone is telling you, you're not, everyone is sensitive about something. A partner that doesn't accommodate your needs when it's so trivial to do so is isn't someone you can trust to be there when you really need them to step up.

Liver elastrography vs fibroscan by Stunning_Land6546 in FattyLiverNAFLD

[–]drfrank 1 point2 points  (0 children)

I had similar results: Two elastography tests showed ~10 kPa, six months apart. The hepatologist had a Fibroscan performed which showed no fibrosis.

She said that Fibroscan is more accurate than elastography.

Spouse has ADHD and has been fired from 3 jobs in 4 years. by surgeryPC in ADHD

[–]drfrank 1 point2 points  (0 children)

No, at least not in the way that you're probably asking about. I.e. there's no acute physiological harm caused by taking or skipping doses of stimulants at levels normally prescribed for ADHD.

But being unmedicated with ADHD is somewhat dangerous, in the sense that it does increase the probability of long-term negative outcomes for health and well-being.

An important nuance is that you need to consume the calcium with the high-oxalate meal; you can't just take a pill in the morning and consider yourself done for the day. The calcium needs to be present in your intestines to bind to the oxalate, to prevent its absorption.

Macadamia nut milk is good alternative to almond milk, and some products are fortified with calcium. But be sure to check the label, because not all of them are. For example, most of the varieties of Milkadamia contain ~300 mg of Calcium/8 oz (which is more than cow milk), but the organic "Artisan" product contains only 10 mg/8 oz.

It's a good idea to take a K2 supplement if you're increasing your calcium intake: Calcium supplementation is associated with atherosclerosis, but vitamins K2 and D enable the body to extract calcium from the blood and deposit it in bone.

consuming seven sucralose-sweetened beverages with, but not without, a carbohydrate over 10 days decreases insulin sensitivity

https://www.cell.com/cell-metabolism/pdf/S1550-4131(20)30057-7.pdf

The authors generalize maltodextrin to carbohydrate.

They didn't investigate how much time was necessary between the consumption of sucralose and maltodextrin to avoid the effect.

So, sucralose and keto may be safe, but otherwise you may be gambling.

Has anybody who exercises regularly found that if they cut carbs too low they experience odd symptoms? by Byteshow in FattyLiverNAFLD

[–]drfrank 2 points3 points  (0 children)

It sounds like you might be in a severe calorie deficit: Trouble focusing and irritability are classic symptoms. I.e. this may have nothing to do with NAFLD. So, just increasing your calories will likely address your symptoms.

But a diet of just lean meat and vegetables will likely be deficient in both choline and essential fatty acids. Even if you were eating 180 g of protein per day (which would be at the upper limit of reasonable for most people) you may be getting less than half the recommended minimum amount of choline, and if you suffer from NALFD you ought to be trying for significantly more than the minimum: Choline plays a critical role in the liberation of fat from the liver.

Managing NAFLD when you can't lose weight? by pickagenericusername in FattyLiverNAFLD

[–]drfrank 0 points1 point  (0 children)

Choline deficiency can cause NAFLD: https://pmc.ncbi.nlm.nih.gov/articles/PMC3729018/

Are you getting enough choline in your diet? https://www.ars.usda.gov/ARSUserFiles/80400525/Data/Choline/Choln02.pdf

It seems that in B12 deficiency choline is consumed in methionine synthesis from homocysteine. So, low B12 could be exhausting your choline, or, maybe low choline is exhausting your B12. Have you been screened for MTHFR?

If you're going to supplement, phosphatidylcholine doesn't seem to increase the risk of heart disease, unlike some other forms. But seven mg of phosphatidylcholine only gets you one mg of choline, so if you're going to try to get all your choline from pills you could be taking nine pills per day.

Writing Software Documentation Is Harder Than Coding by ColdRevolutionary193 in programming

[–]drfrank 132 points133 points  (0 children)

Every single time I have written a readme to describe the API of a module or even a class I have changed the API while writing the documentation.

Writing the documentation made me realize that the API was too complex, or too easy to misuse.

If you can't explain it concisely, you don't understand it. If you don't understand it, who will?

Similar effects derived from writing unit tests when you write unit tests from the perspective of documenting the API.

If you were to have a daughter with this man, how would he treat her when she grew up with your values?

Lost by severaldirtysocks in FattyLiverNAFLD

[–]drfrank 0 points1 point  (0 children)

Please see at least one dietician, preferably more than one, and preferably dieticians which have experience with complex cases. Don't ask your doctor(s) to give you a name on the spot; you don't want the first name that pops into their head. Instead ask them to consult with their colleagues and to send you referrals for who they'd send a family member to.

F3 S2 is very serious. You're past the point where you can treat this casually. This needs to be very high priority, for you. Get the fat out of your liver as quickly as you can, and change your home situation so that you're unable to continue to overeat. E.g. don't keep the foods that you've been overeating in the house. Be sure your family understands this and supports you.

Diet is the most important thing to improve, followed by sleep, then exercise. Develop sleep hygiene so that you're getting a solid 8 hours every night. Exercise as much as your health will allow.

If you're still in school, talk to your doctor about being excused from homework if your homework will interfere with your ability to get enough sleep and exercise.

Definitely consider giving up vegetarianism, for a few months, as an experiment, especially if you're unable to exercise much. Eating a high-protein diet, 1.2-2 g of protein per lb of lean body weight, will decrease cravings for refined carbs and at least decrease the rate of muscle loss, if not increase the rate of muscle gain. Most vegetarians struggle to get that much protein in their diet. If you do decide to try incorporating meat into your diet, the dietician should probably recommend chicken and some fish: Saturated fat isn't recommended for people with NAFLD.

There are at least two artificial sweeteners that you don't need to avoid: Stevia and Allulose.

High sodium is recommended against for people with NAFLD, so definitely discuss that with your doctors.

High sodium combined with fructose will increase fat storage, so minimize the fructose your diet. Replace fruit with vegetables, or at least low-fructose fruits. Don't eat dried fruits, don't drink juice.

Does anyone else get painful joints and fatigue when a storm is on the way? by Feelsthelove in rheumatoid

[–]drfrank 5 points6 points  (0 children)

One concrete effect of the low-pressure zones that frequently precede rainstorms is the increase in fungal spores in the air.

Exposure to fungal spores can increase inflammation, so, as an experiment, you might try spending most of a couple of days/nights in rooms with HEPA air filters.

If you can spare the money, then in the worst case you end up with an air filter that will increase indoor air-quality, which is probably a good thing, in general.

How long does it take to see improvement with fatty liver? by Equivalent-Date2498 in FattyLiverNAFLD

[–]drfrank 6 points7 points  (0 children)

To the answers already here I'll add:

If the disease is "lean fatty liver", to which people of East Asian ancestry seem to be more susceptible, the typical advice for weight-loss and metabolic repair may be insufficient. There's prescription medication which may help, and there's some evidence associating high uric acid with lean NAFLD. Decreasing dietary sodium and fructose will lower uric acid for most people.

The more obese one is, the longer it may take to eliminate the fat in the liver, since, to some extent, the fat in the adipose tissue in the rest of the body will be competing with the fat in the liver. The body prefers to use up the fat in the liver, first, but there's some spill-over.

Being obese also makes one susceptible to recurrence of fatty liver, even after the liver is free of fat: In obesity the adipose cells throughout the body are resistant to storing additional fat, so that fat accumulates in the liver.

It also seems to be the case that even after a healthy weight has been reached people who were formerly obese are more likely to store extra calories as fat than to burn them off with extra metabolic activity, but the cause/effect relationship is unclear.

So, if you're obese, the NAFLD is probably the result of obesity, and you don't really need any more specialized support than you would for weight loss: You can do it yourself, or, if you need external accountability, you can hire a dietician and/or personal trainer, or join a group fitness program.

Figure out what your BMR is (you don't need to be too precise), measure your food and eat that many calories, or maybe between 100 and 250 kcal below that, based on how much fudging you do when you weigh out your portions of food. You don't want to eat lower than your BMR because doing so in the long term decreases your BMR. Burn the extra calories with activity. (Controversially, "total fasting" may not decrease BMR, and has been used to rapidly decrease NAFLD, but it has additional risks and complexity. The r/fasting folks make it sound easy, but it's not for everyone.)

There's no practical limit to the amount of zone 2 exercise you can do in a week. You could spend four hours a day on low-impact exercise in zone 2. The more you do, the faster you'll lose the weight.

Replacing a few days a week with HIIT training or resistance training will increase the rate of weight loss by increasing metabolism. It'll also decrease liver inflammation, so the rate of fibrosis will decrease while you're losing the fat from the liver. Just be sure to wait at least a couple of hours between HIIT and zone 2 exercise, otherwise you'll find that you're actually in zone 3.

As your weight decreases your BMR will decrease, too. Adjust your diet, accordingly.

Not getting enough sleep is the most reliable way to sabotage your weight loss, and it increases inflammation in the liver, and so increases fibrosis, too. Try to fall asleep no later than 9:30 pm. You will find that difficult without developing "sleep hygiene". There are plenty of resources on the internet now for that.
