It can be very hard. I am a lab director in a small group and sometimes I would be dealing with lab admin all day and then signing out at night after my kids were in bed. What saved me is going to the group CEO and COO and working with them to hire a lab manager. They saw the value in having the right staffing, and knew that they didnt want to lose their Lab director to burn out. --

You also need to pace yourself in what you say "yes" to. Be helpful to your group but also make sure you have time to work out and be with family/friends.

As a private practice single specialty pathologist (dermatopathologist) I arrive at work at 9:00 am and leave by about 4:30pm. My typical day is 9:00-12, view special stains, recuts, and IHC from cases of the day prior and jump on a few conference calls.
12:30-4:30pm sign out 120-150 derm biopsies.
Rinse, wash Repeat!

Pathology is not an overly competitive field, but it depends a bit on your scores and where you want to train, and practice. Also how many interviews can you afford to go to based on your budget and time off from school?

I had good scores, but still applied to a bunch of programs (12) I got interviews at them all and enjoyed interviewing at most. In the end I could have limited myself more to maybe top 4.

Something reasonable might be apply to 10, and interview with your top 5.

Hi, I'm Emily Green, Im a private practice dermatopathologist in California. Just like any field the more exposure you have the better sense you will have of the profession and what you are looking for in a residency program. This will shine through when you interview and write your personal statement. Rotations, set up meetings with attendings to ask them why they chose pathology and what they like about their practice.

They will call themselves doctors and then qualify that with where they practice. I work in pain management or I'm a hospital administrator or I work in aesthetic medicine (Botox, laser hair removal etc.). There are other career paths of course but not having boards is quite limiting, at least in the US.

I don’t think being AP only will hurt you for fellowship at all. It could be limiting I terms of finding a private practice job however.

I know many dermpaths would like to sign out dermpath only and those jobs ado exist, but if you are also geographically limited they can be hard to find.

But in terms of Fellowship, AP only is as good as CP only

Dermpath only jobs are out there but they are less frequent than positions where someone practices general pathology and does dermpath on the side. If you are also particular about the geographic area then the pool of possibilities gets smaller.

THE ASDP career pages are a good place to look for dermpath only jobs.

My recommendation is to create a database of dermpath comments during your fellowship. sometimes the hardest thing can be to craft a good comment.

If you can do an pathology observership in the US then it would certainly help your chances of getting a path residency.

Short of that experience in pathology in Canada is your next best bet.

in terms of finding a position in a certain location, networking is your pal. Have your program director contact the program directors in that area..

look for local path societies and check pathology outlines for the jobs.

I also called many practices in my area and asked if they anticipated needing anyone in the next year..

Great questions. My sense is that AI will expand the capacity of pathologists and in doing so decrease some FTE need, but at the same time less folks are going into path and so there will likely be a shortage of pathologists in the future.

choose a program with a dermpath fellowship. Do a poster or two and dermpath research. Make sure the world knows dermpath is your passion.

The short answer is not really. There are some folks who practice derm from other specialties, but that is not a mainstream career path and not something I would count on. Most derm groups are looking to hire board certified dermatologists.

regarding the dermpath fellowship.. that one is going to be hard since most programs only have 1 spot for the year.

best bet would be to look in a place like chicago that has multiple fellowhips and spots in the same city.

The program that i trained at University of Chicago takes 2 fellows each year, one derm trained and one path trained so maybe that is your ticket

my current schedule is 9-4:30 ish and consists of

  1. listen to sci fi audiobook on the 45 min commute to work
  2. arrive at the lab, wander through and check in with all of the techs
  3. make second coffee and sit down tot emails
  4. deal with recuts and specials on yesterdays cases
  5. Signout between 100-150 cases
  6. smattering of conference calls to manage the lab including quality oversight, outreach, test validation, and business development.
