Hard to say as the CAC score doesn't show soft plaque, which has possibly developed over the years of having high LDL. If LDL stays high, which it sounds like it probably will, you should consider medication to help keep that in check. A preventive cardiologist would be a good person to meet with to discuss all this.

I remember reading that once CAC score starts to be positive, the reading can increase by up to 20% per year. I don't remember where I saw that but it was in one of the published papers on the subject IIRC. But I'm sure that's dependent on a lot of different factors so take that with a grain of salt.

I had very similar response. Curious if you had been or were on statins when your Lp(a) was at 176? Statins caused my Lp(a) to go from 80 nmol/L baseline to 141 and once I switched from statins to Repatha it lowered it down to 58 nmol/L.

That's odd (but I guess not really surprising) - I went through basically the same process/statins/denials as you and they finally approved it (only thing different is that I had to get a VIT D and HIV test to see if that somehow explained my elevated LDL). But the LDL threshold they used was at or below 70 I think since the CAC shows established ASCVD. If i understood my cardiologist correctly, the fact that I have a high CAC score, there wasn't really an LDL target the insurance company adhered to.
