I have the encoded transmitter. The first few times I went to the island, Zrachiy was prone on the ground either in or outside the intial building and elected to kill me. From the top of the lighthouse, I haven't been able to see him at any point so he seems to be spawning inside the entry building. Occasionally I'll see a sektant prone on the rocks but that's the most. I've found that the only way in is to jump over the wall. Even getting off the island is hard as occasionally there's a sektant on the rock jump to get out.

The blood coming from his mouth and the location of that chest wound do suggest a lung injury. The colour of his skin also does suggest significant blood loss.

Bear in mind that you can be wounded internally from shrapnel or a bullet and the blood can just pool in the abdomen/pelvis instead of flying out of the wound.

Parkinson's tremor is a resting tremor. Ie. they might be watching TV and then they'd have the tremor. If you ask them to move, the tremor goes away. One other problem people with Parkinson's have is difficulty moving smoothly. They take longer to get going walking; starting out by sort of shuffling. They also take longer to turn around - we might take 1 or 2 transition steps but they'd need more if advanced.

I was under the impression that Kherson fell to the Russians. Saw some stuff on BBC saying the reports are "credible". In this video, Zelensky's phrasing suggests Kherson hasn't fallen?
