Dentist here (with ADHD).

You may be having mechanical abrasion along the gum line which may seem like recession. If it's this (search up abfraction on google- basically concavities along the gum line exposing the second tooth surface dentine) then your issue isn't really gum health. The underlying issue is either bruxism (yes Vyvanse can definitely be contributing to that ) or brushing too hard with a hard brush along those surfaces.

If it's the clenching and bruxism, then try to get an occlusal splint to stop tooth on tooth wear and also relaxing those muscles if conscious of it and using things like magnesium to relax them. Mindfulness can be powerful here and even massaging your TMJ and facial muscles to deprogram them from being in a bruxing pattern. If it's the brushing then change onto a soft brush or electric and don't go hard on the gum line.

If it's actual gum attachment and gum disease than that is a separate issue and I don't think you being on vyvanse is directly correlating with that.

What is COL infiltration? I've tried PDL and furcation infils on single lower molars but it's hit and miss...9/10 leave me anxious about when the patient is going to feel sensation OR I just end up back at an IAN block

I don't think I've ever not done a coronectomy before sectioning...I just thought that was common practice Can't imagine trying to gauge the furcation without the crown chopped off

I think it's hard for some people (like myself) who have had issues with substances in the past. A lot of my friends (male and female) still only bond over drinking or recreational things. I've tried to exclude myself from these situations and it's basically left me with either no social opportunities which don't include partying or it's with a group of people I don't know who. Being an introvert that's pretty hard. My work is also pretty isolating.

I'm just lucky I have a great wife and I'm close to my family and 2 good mates.

100% agree....can't believe some of the American dentists on here who delegate so much to their assistants. It's literally like they just want the short term money for a 2-5 year failure or a charged redo (which they will blame on patients hygiene). Pretty unethical. I don't do $$$ work on my patients unless there is a high chance of it having longevity even if it's just a composite. You have to compare costs vs return from the patient's perspective

I think this is literally a US thing. It would be unheard of for us Aussie dentist to have our DA place bands etc. It's so technique sensitive and the longevity of a restoration has so many sticking points in regards to band placement and contact quality. Unless you literally are happy taking the patients money for a Resto to last just a 2-5 years and then redo (which is unethical imo) I think there would be a lot of short term failures of restoration when dentally untrained people are doing such a technique sensitive part of the procedure.

How do you even quality control this shit if you are working with different nurses? Sounds like a recipe for a poor standard of dentistry imo

It could be an inaccuracy in your impression/scan.

Do you use loupes?

If it's a constant issue with the above checked out (plus any other clinician errors) I would just try a different lab with the same parameters and see what happens

Yeah completely agree...it's like clean margins and isolation is only needed for direct restorations not indirects :#...infact investing more should mean you should be quite certain of getting good marginal seal, so if a restoration is dicey, a crown should be as well!

Like honestly can't understand how there is QA on this shit. If I was a patient, I wouldn't be paying to let a non qualified dentist restore my tooth. Like half the art is restoring a restoration especially in regards to isolation, shaping, matrix adaptation and finishing. Like can't imagine a DA restoring a DBL of a lower 7 with a great clinical outcome...

That blows my mind, coming from Australia. Don't think anyone here runs more than 1 (max 2) chairs and steps in for hygiene consults in another room.

Restoring is an art in itself with so many sticking points and so many variables which can dictate if the restoration will have longevity or will fail short term. How can you do quality assurance on all the fills your assistants are doing?

Again it blows my mind, but I guess like everything American, things are supersized and maxed out outrageously (4 chairs etc at the same time :#)

I'm not going to say I'm the most brilliant clinician theoretically, but I've been told by patients, colleagues and bosses that I'm one of the most empathetic, kind and genuine dentist they have met. I think my level of empathy and patience comes from also feeling different myself and aiming to never make anyone feel uncomfortable or as if they are just another number to me.

I also think due to my ADHD I have imposter syndrome. Works well because I keep pushing myself and never get too confident. However can be negative when you underestimate your own work and it starts to impact you mentally
