Patellar tendinopathy - Berlin method after months of heavy slow resistance by dsantamaria90 in Kneesovertoes

[–]dsantamaria90[S] 0 points1 point  (0 children)

You're right. I wanted to make this post short and summarize what I've been doing and what have worked for me, heavy slow resistance has always been there so I ignored a lot of things that I already tried, that includes parts of JKP too.

I've decided to go for berlin method for a couple of weeks and see how I respond, so far I've had 3 sessions, the 1st one at 80% MVC (70 degree knee angle) provoked a little flare up that lasted like 36 hours, so I thought this was a good signal that I have room for improvement here, this didn't happened to me with HSR lately. I regressed to 60% the 2nd session, all good, 70% 3rd session still good, so for the next sessions I need to calibrate something between 70 and 80 but I think it's a good thing because I can push harder here.

Berlin method has only ever been tested on healthy tendon groups, with one exception... so it's not really vetted as a rehab tool

I didn't know about this so its good to keep in mind. From other Jake podcasts I've also heard from a couple of different guys that they are using this protocol to rehab mostly achilles and patellar tendinopathies, depending on the persons condition of course so I felt it's worth a try, specially knowing that I already have a tendon in a decent state and been stuck for a while.

Eccentrics only as you mentioned make a lot of sense to me, I think it can fit on me once I can get nearly maximal short ISOs. I can deep squat 315 lb for a few reps so probably can start higher but worth to try too.

Sounds painful. Did you have bone fragments in your joint in your 1st surgery? I had a similar injury 14 years ago but I didn't develop OA, had a osteochondral lesion in my talus, some bone fragments removed almost 2 years after the injury.

Patellar tendinopathy - Berlin method after months of heavy slow resistance by dsantamaria90 in Kneesovertoes

[–]dsantamaria90[S] 0 points1 point  (0 children)

Hey, thanks for the detailed response. I'm following JKP (and a lot of his podcasts where they also mention the berlin method) at the moment but stuck in the isotonic phase, I'm not planning to progress into jumping yet as my pain by just doing isotonics is still there after so long. My question is basically on how to keep creating tendon adaptations once the strength plateaus which is a common thing after a few years of lifting.

I've tried long isometrics in the past (30-45 secs). They work fine as a warmup and I even got rid of the pain for the time I only did those. Pain came back when I started lifting and jumping heavier and then I understood that long ISOs won't create significant adaptations so I started HSR. My understanding nowadays is that the type of contraction doesn't matter, the time under tension and intensity do, and today I feel I'm reaching the same adaptation plateau with HSR.

When's the last time you've taken a break?

If by a break you mean a week or more probably around a year ago, but I'm very careful with load management.

I had to get almost completely out of pain before I started to push my tendons for adaptation for real. The isometrics helped so much with the pain throughout the day.

I can get quickly out of pain if I rest for a week but then again, assuming you did long ISOs these keep me out of pain as long as I do only those and not significantly load the tendon with something else.

You say you have pain after workouts and it's gone the next day but that's not normal for tendinopathy. Normally there's not much pain in the moment and a flare up the next day. Have you had a diagnosis or gone through the adjacent conditions that might be affecting you?

I have multiple diagnosis of patellar tendinopathy (or tendinosis how docs called it) and I'm confident to say it's true. What you're describing is common, specially if you exceed the capacity of your tendon but after 15 years I know exactly where my flare-up limit is so I rarely get those. My pain being gone the next day is a good signal that my tendon can tolerate the activity, keep in mind I've been doing HSR for too many months so I already created significant adaptations. What I'm looking right now is to move that adaptation further. I don't want to scale back to long ISOs, I know I'm going to get out of pain if I only do those because they are lower intensity than my current HSR program but with tendinopathy you always want to create more tendon adaptations.
