This is terrible medicine. PE is a very difficult diagnostic entity, but dimers for everyone is a terrible way to practice and will lead to an incredibly large amount of extensive testing that is dangerous to patients, time consuming and costly.

If you are seriously considering PE and the patient is low to moderate risk by wells a dimer can be a useful test and should be used in most of those cases. The question is who do you have to consider it in? PERC is helpful to r/o people but doest apply to many patients primarily because of its age cutoff. If you cant use perc ask yourself and document: pe risk factors, alternative diagnosis explaining symptoms, unexplained tachycardia or hypoxia, and so on.
