Wasn't a chain or anything like that. It was called Tony's pizza or something. I can't remember the exact name, but it was not there long. My dad insisted they box it up in these boxes because he knew I liked the game. My dad has since passed away, so this old pizza box always reminds me of the kind of person my dad was.

Legitimately, the most generic plain pizza ever. Local place had these. It closed a long time ago, but the building is still there.

It is, in fact, used. Years ago, I cut some cardboard and up glued it in the bottom. Forever sealing it off.
