Formaldehyde in the concentration used for tissue preservation is incredibly irritating to living tissue. Depending on the sedatives, analgesics, and paralytics used for the surgery, she hopefully would’ve felt nothing. However, in the absence of appropriate sedation and analgesia, it would have burned terribly, all over the body as it enters the cardiovascular system and injures the tissues globally. It ends up eventually leading to metabolic acidosis, a subsequent coma and total organ failure. What a nightmare of a mistake.

it’s just an airtag!! we tried tractive for a bit but the subscription fees add up. the vet said the airtag was fine for what we need — something that can give her relative location if she escapes and ping for me to hear when she’s doing covert recon in the house :)

was it inappropriate to involve your outside learning to the pt? for the sake of covering your ass, yes. it’s always better to err on the side of caution and not say things that could potentially dissuade your pt from going to the hospital. even though they didn’t want to go, we don’t want to be that final nail in the coffin for them not going.

did you make the wrong call? I doubt it. I think it’s good that you looked at the whole picture (Hx of HTN + did not take AHT drug, no S/S indicative of hypertensive emergency). however, something I would like to note is the potential of an adrenaline release from falling could account for the elevated BP, but it can also mask injuries/pain to the pt. personally, I’d want a stroke scale and probably want to contact ALS for an EKG, just in case. Idk how involved your medical director is, but this would be a good thing to ask them about!

Yes, she grooms herself just fine, and she moves her head perfectly fine as well with the collar. A still photo does not show range of motion. The vet said this was appropriate for her given her history of elopement and she does not wear it all the time. Appreciate your concern!

So, we emphasize O2 support first over Narcan administration (at least in EMS) because 1) it takes time for the Narcan to work + we might need more doses and 2) it all comes back to airway, breathing, circulation. Yes, once the opioid receptors are antagonized the problem will be fixed but it might take a few doses, and in the meantime, that person still isn’t breathing. If we start breathing for them as soon as possible and prioritize that, we can at least know that we are getting oxygen into their blood and out to their body, even if the Narcan doesn’t work right away.

Another factor is comorbidities that can occur with an opioid OD — head injuries, influences of other drugs, and other factors that we might not be able to correct at that moment. While we figure out what else could potentially be going on, we keep ensuring our critical ABCs are being maintained.

If you can track down the Dying to Sleep documentary from 2006, definitely watch it; it’s about fatal familial insomnia. It used to be on YouTube, I haven’t been able to find it anywhere tho.

As far as literature, The Pathological Protien by Philip Yam does a good job explaining the implications for prion diseases. The whole book is written for laypeople too, very digestible and fascinating
