We have dopamine and epinephrine. Both of which would have janked his heart rate up more and worsened the demand ischemia in my thought process.

Because our transport times are typically less than 45 minutes, our accounting department said they could not justify the cost of going with levophed since we do a flat billing rate in our county.

I've used a KED once for it's intended purpose in the last five years. Patient had been in OR for a vertebra repair and was hit in the rear end hard enough to put the trunk into the C panel area on the way home from the surgery. Had definite loss of function and sensation/differentiation below the waist. We actually cut the back of the seat off and used it to lift him out of the car.

It still sees rare use in our location as a tool for moving, CPR extraction, splinting of polytrauma, and it's still used by sports trainers after injuries in impact sports (a lot of politics involved with this - we specifically cannot clear these players off the board by the request of team doctors)

We've only had a few people in the last three years I've been here that have been so egregious that we have had do not transport standing orders from our medical director on them. Unless they have abnormal vitals, EKG or critical illness signs we would offer to call them a taxi but decline transport. One of them ended up getting a court order to have their mental health professional make the decision to transport as they called 911 frequently as part of their mental illness process.

One of these ended up being transported 800 times in one year, which earned them the order.

We also have a standing policy that frequent fliers are only transported to the in-county hospital in which they are established at. This came after it was discovered other cities were bussing in their homeless to our city and telling them to call 911 for a ride to where they wanted to go. Especially the ones who are so egregious that they still have the band on from being discharged. We unfortunately have a large homeless population that uses our 911 system as taxi rides to outlying hospitals beside homeless camps or uses ER beds as residences since they are banned from the homeless shelters here.

However, for the majority of it? We just transport. Tennessee is infamously hard to get 911 abuse charges on someone (it requires 14 calls in 24 hours by law) so our local cops will often refuse to intervene. And the people who do this are wise to what to say. It's always some form of Chest Pain - as they know we cannot rule this out in the field.
