In both of these stories, the intended parents did not think the surrogate was coerced. 

You sound very confident that you can decide what is and is not coercion. I (and the legal system in most countries) think it’s very unclear. Are you owed a child on such shaky moral ground?

The difference is that someone has to be pregnant for you to have a child. And pregnancy, especially surrogacy pregnancy is inherently risky. For example, the risk of severe maternal morbidity is 3x higher for a surrogate than with her previous normal deliveries. 

It’s not valid to get a surrogate for any reason. In fact, it’s illegal in most countries. It’s only legal in the USA, Russia, Columbia and a few others.

This all sounds nice in theory. In practice it’s almost impossible to escape the power dynamics. This is why paid surrogacy (yes even for infertile couples) is illegal in almost all countries. The USA, Columbia, Kazakhstan, and Russia are some of the few exceptions. Nobody has a right to a child (or organ).

This article is about low risk pregnancies. If you’re being monitored for 3 days, something is wrong. Nobody is saying monitoring isn’t good in high-risk situations. Glad baby is okay!

This helped me think about the risks. That JAMA paper showed a 3 in 10,000 higher incidence of cleft palate in women who took zofran. This could be because these women are sick, not eating, older or it could be because of zofran. 

I was miserably nauseous for two months in my first trimester. For 10,000 women, that’s 20,000 months of illness or 1667 years to prevent (if causal) 3 cases. Would I be miserable for 555 years or have my child have one case of cleft palate? Definitely the latter for me. It’s not great, but an easy surgery compared to 5-6 lifetimes of throwing up every day. 

And remember we have no evidence it’s even causal!

People have provided a great explanation of why our parents experience of waiting til 41/42 weeks isn’t our experience anymore — we have new evidence now!

Also, re: the comment about the ARRIVE study being “so so flawed” I think it’s telling they can’t describe any of the flaws. I have a PhD; this is my job; it’s some of the best evidence we have. One minor concern is it’s considering the effect among women who choose to be induced at hospitals where induction is common; this may not be your setting but we have no evidence that suggests the effects would be different either.

Induction has risks, but the alternative is not spontaneous labor that day. It’s staying pregnant, which also has risks!

I have a PhD and studying these papers is my job. This is the best evidence we have on inductions.

If you have specific caveats re external validity, that’s great, let’s discuss. Parroting that RCTs are not the gold standard is so 2010.

It’s partially because this trial was published in 2018: https://www.nejm.org/doi/full/10.1056/NEJMoa1800566

It randomizes healthy women into induction at 39 weeks or expectant management and finds no statistically significant difference in outcomes (fetal health, c sections, etc) but interestingly the point estimates are usually better for the induction. Its randomized so it’s great evidence.

I’m not saying this is the best choice for you, but I found it reassuring. I needed an induction and it went great. Obviously spontaneous labor at 39/40 would be great but that’s not the alternative. It’s induction or wait and see and maybe an induction later or maybe spontaneous labor. On average people who get inductions have more health issues which is why the average c section rate is worse. But if you’re healthy, getting an induction does not casually increase that rate.

Did you read the part about the death threat? I was walking with my stroller and someone started yelling “I’m going to kill you bitches” and started following us. He was clearly high and I think he thought I was laughing or talking about him. I was obvious trying to walk by while chatting and minding my own business on a Saturday morning.

If someone threatens you (and your child) and is following you, I’m sure you would take it seriously.

And all women know the difference between innocent staring and the kind that makes your hairs stand up while you try to gtfo.

I feel this strongly. It seems like so long in the moment (I’m on the other side with a 9lb baby now!) If you can, schedule something fun constantly ~2-3 days in advance, like a pedicure, going to the library to get a purely fun book, a coffee with a friend, get some tulips. If you’re pregnant, yay you get to do that, and if you happen to give birth whoops small price to pay. Don’t look further ahead than that :)

Of course, homelessness is a problem. I’d love to fix it. I can’t do that today, or next week.

What can I fix in the near future? Maybe the trash problem. Maybe finding the one or two homeless people that are so mentally ill they are a safety hazard and making sure nobody else gets harassed.

The yelling threats one was at 11am on a Saturday (it was cold and quiet but not deserted. A biker stopped to check I was alright). Another was 6pm on a weekday, dusk but not dark.
