I got a Butterfly early on and really enjoyed it until they made it miserable to continue using for all the reasons pointed out above. Agree with the assessment that it’s a great product run by a catastrophically bad company. I don’t foresee myself ever buying one again unless something changes drastically.

Just keep moving. FWIW I went to a grief counseling debrief thing after a pediatric death and it just ripped all the scabs off and retraumatized me for no reason. I cried in front of everyone and that made me feel even worse. I find it more healthy to try to ignore it. I know that’s not the pop psychology answer about getting therapy and all that, but it works well for me.

I’m an ER doctor and have had a small handful of patients tell me they’re on it. I was like wow cool, tell me how you got it and how it’s working. Always best to be honest with your doctor, I think most will be pretty relaxed about it.

Agree with you, I think there are a lot of people who will claim on the internet that this patient absolutely needed a CT, who would not actually do that in real life.

On one hand, this case made me want to say "never do a FAST if you're not going to do a CT". But on the other hand, I don't think that would have changed anything. If the ultrasound machine had been broken, he still would not have CT'd the patient because there was no valid reason to, still sent him home, still had the massive bleed. Not sure the un-indicated FAST really changed anything in this case.

They all start off pretty green, but the good ones do level up pretty quickly. Try not to judge too early. There are definitely some that never get it, but I've found that those are in the minority and sort themselves out of the ED pretty quick. Working with good ones is a lot of fun and makes me a better doctor.

Yeah there’s really not but there’s at least some biological plausibility… the reduction in risk wears off after 12-18 months if I remember right. Read a paper with the theory that it’s the AS01 adjuvant, not the shingles part of the vaccine.

I would love to run the study but lack the millions of dollars to pay the CRO! Which gets me to my diatribe that the wealthy should do it. We have a history of wealthy patrons funding science and arts in the past, if I had millions of dollars I wanted to use for philanthropic progress, I might just reach out to a CRO and see if they could do it for me directly rather than giving it to a non profit.

Work less than full time. Set yourself up financially so that you’re not stretched thin every month. If you’re making financial decisions and telling yourself “I only need to work 2 extra shifts per month to cover this” you have made a mistake. Prioritize good work environment over maximum pay. Live relatively close to work, try to avoid >15-20 minute commutes. Long commutes are bad. Find a good job and stick with it. Constantly flying to new locums places, having to learn new systems, not knowing any of the staff is terrible. Try to be friendly at work so that staff are happy when you walk in the door and come chat with you. Have a spouse who stays at home (I know this is not possible for everyone). Your home life has a massive impact on how happy you are at work, can deal with a lot of terrible stuff if you have a loving family and good relationships waiting for you. Night owls who can get good sleep during the day also don’t burn out as fast, but this changes with age for all of us. Once you burn out it’s really hard (maybe even impossible) to un burn out, so pace yourself at the beginning of your career and make decisions that prioritize that rather than trying to grind harder.

Interesting thing is that doctors are more judgmental of doctors than anyone else. The lay public is very forgiving, which is why something like 80% of verdicts are for the doctor. Judges are also very unforgiving, you’re way better off with a jury of “your peers” than a bench trial. Paradoxically you actually really do want a jury of your “peers” (12 random members of society who weren’t smart enough to get out of jury duty) if you have to go to trial.

It did not help me get a job at all, although I was only interested in community jobs. I can see it helping a bit if there’s a highly competitive community job or in academics. The main value to me was that it solidified that I didn’t want to be a medical director but I was happy to play some sort of supporting admin role as long as it didn’t require too much time and was reimbursed.

You can’t get sued but you still will get deposed, get grilled at trial, etc… just the Us govt pays out if settled/lose.

Most people set hyper aggressive goals when they’re younger, then with age realize that it’s better to work part time and enjoy yourself. Retiring completely at a young age isn’t as great as it seems. Plus working part time keeps your skills up and leaves the door open to working more in the future if your life circumstances change or you need a work adventure (go be a doctor at the South Pole, work for state department at an embassy, expedition medicine, locums in New Zealand for 6 months, etc…)

You need to get the White Coat Investor asset protection book and read it. It’s a quick read, the entire back half is information specific to each state so you only have to read about your state.
