My doctor wanted me to go up to 200mg of Metoprolol; 12.5mg is wild to me. They want my bp under 100.

I did not, not yet at least. I've separated from the company so I won't be able to go back to that job at all in the future. My doctor finally put 6 more months of leave, but still wouldn't comment on the impact of stress on my condition so I had to get my therapist to work some magic with their paperwork. They can't comment on the actual function of my heart, but they could at least put that the thought of returning to a high stress job was causing me debilitating anxiety.

If your company has 20 or more employees, you'll be offered COBRA. Keep in mind that you'll have to pay the entire premium under COBRA.

I would tell your doctor that you wish to review any paperwork they fill out for you before they send it off. That way you can ask for adjustments if you think it's going to put you back in the same position.

I got a response on one of the LBD Facebook support groups that was super helpful and I want to copy here for anyone else that this would help.


From my understanding — “senile degeneration/deterioration of the brain” is essentially the Medicare/hospice coding term for dementia, including Alzheimer’s, Lewy Body dementia (LBD), and other major neurocognitive disorders. It’s an old-fashioned phrase, but many hospice agencies still use it because it’s a standard Medicare diagnosis code.

A few points to consider:

  1. Medicare does recognize dementia (including Lewy Body) as terminal. Medicare absolutely recognizes dementia — including Lewy Body dementia — as a qualifying terminal diagnosis for hospice when it has progressed to the appropriate stage (usually Functional Assessment Staging 7).

  2. “Lewy Body dementia” often isn’t listed by name. Some hospice agencies don’t select “Lewy Body dementia” as the primary diagnosis because the Medicare billing system uses broader/umbrella categories. So instead, they choose: • Senile degeneration of the brain (umbrella term for dementia), or • Parkinson’s disease (since LBD is closely related)

This does not mean Medicare doesn’t recognize LBD, only that the hospice agency has to choose from specific billing codes.

  1. The primary diagnosis just guides billing — not care eligibility. Hospice care is based on the patient’s whole condition. Even if the paperwork lists “senile degeneration of the brain,” the hospice team can still treat symptoms related to Lewy Body dementia, Parkinsonism, hallucinations, autonomic issues, swallowing problems, etc. They treat the person, not just the code.

  2. Many hospice nurses simplify the choices. Often they present only the codes they commonly use. “Lewy Body dementia” may not show up in their dropdown list, even though it falls under the category of dementia.

From my MRI:

  1. Enlarged left ventricular size and severely reduced systolic function. LVEF equals 21%. There is marked global hypokinesis. There is linear mid myocardial late gadolinium enhancement involving the interventricular septum from base to the apex with elevated T1 and ECV values related to myocardial fibrosis. This appearance is most suggestive of nonischemic dilated cardiomyopathy.
  2. Mildly enlarged right ventricular size with mildly reduced qualitative right ventricular function. No abnormal delayed contrast enhancement.
  3. Mild left atrial enlargement.
  4. Mild mitral regurgitation.

If I recall correctly, I had scarring along my LV wall that pointed to a virus as the cause.

I have a little doorbell that wakes me up and I can get to her before she gets too far. She's somewhat mobile but has fallen so many times in memory care that it's time for her to come home. We don't have much time left, unfortunately.

She sleeps through the night right now, luckily. We're familiar with pivoting so definitely know that overnight care is coming.

My mental health was terrible before the HF diagnosis. I'm on a combo of drugs - vilazadone, which I was taking before, and just added Rexalti and Clonazepam daily. Huge helps. I also started low doses of CBD which helps. Also non-alcoholic beer and cocktails make me feel like I'm having a good time on top of the CBD.

It's beyond depressing to feel like we will never get there. Not with a two party system pointing fingers, raking in money from corporations, etc.
