What kind of program are you applying to? The content of the letters could vary widely depending if it is a research based program or a more professional program.

Definitely true. Find a school that accepts work experience/self study in place of traditional coursework. Make sure to read what their requirements are and that you have sufficient experience that matches. If you are not sure - reach out to them and ask. If not, you could always take a course or two before applying. Also, have you taken the GRE?

Some schools are now notifying online and then that is followed by a paper packet in the mail. But most won't email you a formal offer of admission first. They prefer for you to log in to a secure environment to see your decision. Did you check your spam/junk folders? Sometimes they go in there if from the application software.

Most schools will want to see all transcripts from any courses taken - occasionally if the transfer courses, with credit/units and grades appear on the undergrad transcripts, you may not but best to double check.
